目的:探讨血清尿酸(SUA)水平与慢性肾脏疾病(CKD)患者心室肥厚的相关性,尤其是这种相关性是否存在性别差异。方法连续性入选2010年4月~2013年10月就诊于我院的216例CKD患者。使用超声心动图评估左室质量指数及左室肥厚(LVH)情况。结果左室肥厚的发生率为男性58%,女性47%。在多因素回归分析中,男性的SUA水平与左室质量指数无关,而女性的SUA水平与左室质量指数明显相关(β=0.27,P=0.02)。同时,对于男性而言,糖尿病(OR=4.41,P=0.01)与左室肥厚相关,而对于女性患者,年龄(OR=1.13,P<0.01)、高血压(OR=7.38,P=0.03)和 SUA(OR=1.91, P=0.03)均与左室肥厚相关。结论女性CKD患者SUA水平与左室质量指数和左室肥厚相关,而男性患者未发现这种相关性。女性患者SUA水平与左室肥厚之间的相关性比男性更明显。

목적:탐토혈청뇨산(SUA)수평여만성신장질병(CKD)환자심실비후적상관성,우기시저충상관성시부존재성별차이。방법련속성입선2010년4월~2013년10월취진우아원적216례CKD환자。사용초성심동도평고좌실질량지수급좌실비후(LVH)정황。결과좌실비후적발생솔위남성58%,녀성47%。재다인소회귀분석중,남성적SUA수평여좌실질량지수무관,이녀성적SUA수평여좌실질량지수명현상관(β=0.27,P=0.02)。동시,대우남성이언,당뇨병(OR=4.41,P=0.01)여좌실비후상관,이대우녀성환자,년령(OR=1.13,P<0.01)、고혈압(OR=7.38,P=0.03)화 SUA(OR=1.91, P=0.03)균여좌실비후상관。결론녀성CKD환자SUA수평여좌실질량지수화좌실비후상관,이남성환자미발현저충상관성。녀성환자SUA수평여좌실비후지간적상관성비남성경명현。
Objective To determine whether SUA being associated with cardiac hypertrophy in CKD patients ,focusing on sex differences. Methods 216 CKD patients were enrolled in this study. Left ventricular mass index (LVMI) and left ven-tricular hypertrophy(LVH) were assessed by echocardiography. Results The prevalence of LVH was 58%in male and 47%in female. In multivariate linear regression analysis,SUA levels did not correlate with LVMI in male,whereas SUA was indepen-dently associated with LVMI in female (β=0.27,P=0.02). Multivariate logistic regression analysis also revealed that diabetes mellitus(OR=4.41,P=0.01) was associated with LVH in male,whereas age (OR=1.13,P=0.01),hypertension (OR=7.38, P=0.03) and SUA (OR=1.91,P=0.03) were associated with LVH in female. Conclusion In female CKD patients,SUA levels were associated with LVMI and LVH,whereas there was no association in male patients. These observations suggest that an asso-ciation between SUA levels and the development of cardiac hypertrophy is more likely in female than in male.
