目的:探讨彩色多普勒超声对甲状腺结节良恶性判断的临床应用价值。方法利用彩色多普勒超声对57例甲状腺结节患者74个病灶进行术前检查,观察患者结节的形态、边界、回声及结界内血流情况,并与术后的病理诊断相比较,以病理检查结果为准。结果彩色多普勒超声检查中,甲状腺结节良性结节患者43例,病灶51个,恶性结节14例,病灶23个;病理结果为甲状腺结节良性结节45例,病灶49个,恶性结节12例,病灶25个。甲状腺良性结节声像学主要表现为形态较为规则,边界清晰,内部为高回声,且结节内血流匮乏;恶性结节声像学表现为形态不规则,边界模糊不清晰,内部为不规则的低回声,且结节内血流丰富。彩色多普勒超声诊断甲状腺结节的灵敏度为82.6%,特异度为88.2%,准确性为86.5%。结论彩色多普勒超声对甲状腺结节良恶性的鉴别诊断具有较高临床应用价值,值得应用。

목적:탐토채색다보륵초성대갑상선결절량악성판단적림상응용개치。방법이용채색다보륵초성대57례갑상선결절환자74개병조진행술전검사,관찰환자결절적형태、변계、회성급결계내혈류정황,병여술후적병리진단상비교,이병리검사결과위준。결과채색다보륵초성검사중,갑상선결절량성결절환자43례,병조51개,악성결절14례,병조23개;병리결과위갑상선결절량성결절45례,병조49개,악성결절12례,병조25개。갑상선량성결절성상학주요표현위형태교위규칙,변계청석,내부위고회성,차결절내혈류궤핍;악성결절성상학표현위형태불규칙,변계모호불청석,내부위불규칙적저회성,차결절내혈류봉부。채색다보륵초성진단갑상선결절적령민도위82.6%,특이도위88.2%,준학성위86.5%。결론채색다보륵초성대갑상선결절량악성적감별진단구유교고림상응용개치,치득응용。
Objective To investigate the color Doppler flow imaging ( cdfi) in benign and malignant thyroid nodule judgment of the clinical application value .Meth-ods Using the color Doppler flow imaging ( cdfi) in 57 cases of 74 patients with thyroid nodular lesions on preoperative examination ,the nodules were observed the mor-phology,blood flow within the boundary,echo and enchantment,and compared with postoperative pathological diagnosis,the pathological examination results shall pre-vail.Results Color Doppler ultrasound examination in 43 patients with thyroid nodules are benign nodules,were 51,14 cases malignant nodules,23 lesions;Pathological results for thyroid nodules are benign nodules 45 cases,lesions 49,12 cases of malignant nodules,25 lesions.Benign thyroid nodules audio -visual learning main show is comparatively rules form,clear boundary,internal for high echo,and the lack of blood flow in nodules;Malignant nodules audiovisual learning performance for shape is ir-regular,fuzzy boundaries is not clear,the internal for irregular low echo,and abundant blood flow within nodules.The sensitivity of color Doppler ultrasonic diagnosis of thyroid nodule was 82.6%,88.2%,86.5% accuracy.Conclusion Color Doppler flow imaging(cdfi)in the differential diagnosis of benign and malignant thyroid nodule with high clinical value,worthy of application.
