目的探讨关节滑膜骨软骨瘤病的影像学表现及鉴别要点。材料与方法回顾性分析15例关节滑膜骨软骨瘤病患者的X线、CT和MRI的影像资料。结果15例中11例经手术病理证实,4例关节镜证实。X线、CT和MRI均可见游离体形成。游离体体在X线和CT上表现为高密度的钙化小体,圆形或椭圆形,大小不一。小结节影在MR T1WI呈低信号,在T2WI信号不均,钙化部分呈低信号,而未钙化部分呈中等或高信号。CT检查可以发现隐蔽游离体、增厚的滑膜和关节腔积液。MRI能发现关节内未钙化的游离体,除可见关节游离体外,还清晰显示软骨、滑膜、周围软组织的改变和关节积液等表现。结论游离体是滑膜骨软骨瘤病的特征性影像表现,是影像诊断的主要依据,X线诊断简便、快捷,CT、MRI可提供更多的信息。
目的探討關節滑膜骨軟骨瘤病的影像學錶現及鑒彆要點。材料與方法迴顧性分析15例關節滑膜骨軟骨瘤病患者的X線、CT和MRI的影像資料。結果15例中11例經手術病理證實,4例關節鏡證實。X線、CT和MRI均可見遊離體形成。遊離體體在X線和CT上錶現為高密度的鈣化小體,圓形或橢圓形,大小不一。小結節影在MR T1WI呈低信號,在T2WI信號不均,鈣化部分呈低信號,而未鈣化部分呈中等或高信號。CT檢查可以髮現隱蔽遊離體、增厚的滑膜和關節腔積液。MRI能髮現關節內未鈣化的遊離體,除可見關節遊離體外,還清晰顯示軟骨、滑膜、週圍軟組織的改變和關節積液等錶現。結論遊離體是滑膜骨軟骨瘤病的特徵性影像錶現,是影像診斷的主要依據,X線診斷簡便、快捷,CT、MRI可提供更多的信息。
목적탐토관절활막골연골류병적영상학표현급감별요점。재료여방법회고성분석15례관절활막골연골류병환자적X선、CT화MRI적영상자료。결과15례중11례경수술병리증실,4례관절경증실。X선、CT화MRI균가견유리체형성。유리체체재X선화CT상표현위고밀도적개화소체,원형혹타원형,대소불일。소결절영재MR T1WI정저신호,재T2WI신호불균,개화부분정저신호,이미개화부분정중등혹고신호。CT검사가이발현은폐유리체、증후적활막화관절강적액。MRI능발현관절내미개화적유리체,제가견관절유리체외,환청석현시연골、활막、주위연조직적개변화관절적액등표현。결론유리체시활막골연골류병적특정성영상표현,시영상진단적주요의거,X선진단간편、쾌첩,CT、MRI가제공경다적신식。
Objective:To evaluate the image ifndings of synovial osteochondromatosis and differential diagnosis. Materials and Methods:X-ray, CT and MRI ifndings in 15 cases with synovial osteochondromatosis were analyzed retrospectively. Results:11 cases of 15 cases were proved by clinical and pathologically, 4 cases of 15 cases were proved by arthroendoscopy. X-ray, CT and MRI can ifnd small loose body, X-ray and CT can find high densidy calcification round and oval shape, large or small of small loose body. MRI can ifnd small cartilage nodular had hypointensity on T1WI, had a mixture of hypo-hypeintensity on T2WI. CT examination can be found hidden loose bodies, thickened synovium and intra-articular effusion MR can detect intra-articular not free of calciifcation cartilage tumor nodules, in addition to articular loose bodies can be seen, but also clearly indicated that cartilage synovial soft tissue changes and joint effusion. Conclusions:Loose body is the characteristic features in imaging. X-ray diagnosing simple and high-speed, CT and MRI can ifnd more information.
