目的:探讨血浆 N 端脑钠肽前体(NT-proBNP)水平与脑出血患者预后的关系。方法选取46急性脑出血患者作为观察组,以同期健康体检者46例为对照组,对比2组患者血浆 NT-proBNP 水平,并分析脑出血不同时期、不同格拉斯哥昏迷指数(GCS)评分、不同出血量及不同预后患者的血浆 NT-proBNP 水平。结果观察组血浆NT-proBNP 水平显著高于对照组(P <0.01),脑出血患者入院时血浆 NT-proBNP 水平显著高于入院7 d 时(P <0.01);GCS 评分≤8分较 GCS 评分>8分的患者血浆 NT-proBNP 水平显著增高(P <0.01);血浆 NT-proBNP 水平随着脑出血量的增加而增高(P <0.01);好转患者血浆 NT-proBNP 水平显著低于死亡患者(P <0.01)。结论脑出血患者血浆 NT-proBNP 水平明显增高,且急性期增加较为明显,同时血 NT-proBNP 与脑出血量、神经功能损伤程度及患者预后均具有密切关系,该指标的检测有助于评估脑出血患者的病情及预后。

목적:탐토혈장 N 단뇌납태전체(NT-proBNP)수평여뇌출혈환자예후적관계。방법선취46급성뇌출혈환자작위관찰조,이동기건강체검자46례위대조조,대비2조환자혈장 NT-proBNP 수평,병분석뇌출혈불동시기、불동격랍사가혼미지수(GCS)평분、불동출혈량급불동예후환자적혈장 NT-proBNP 수평。결과관찰조혈장NT-proBNP 수평현저고우대조조(P <0.01),뇌출혈환자입원시혈장 NT-proBNP 수평현저고우입원7 d 시(P <0.01);GCS 평분≤8분교 GCS 평분>8분적환자혈장 NT-proBNP 수평현저증고(P <0.01);혈장 NT-proBNP 수평수착뇌출혈량적증가이증고(P <0.01);호전환자혈장 NT-proBNP 수평현저저우사망환자(P <0.01)。결론뇌출혈환자혈장 NT-proBNP 수평명현증고,차급성기증가교위명현,동시혈 NT-proBNP 여뇌출혈량、신경공능손상정도급환자예후균구유밀절관계,해지표적검측유조우평고뇌출혈환자적병정급예후。
Objective To discuss the relationship between plasma N-terminal pro-brain natriuretic peptide(NT-proBNP) concentration and prognosis in patients with cerebral hemorrhage.Methods Forty-six patients with cerebral hemorrhage were selected as the observation group,and 46 healthy sub-jects as the control group.The plasma NT-proBNP concentration was compared between the two groups. In addition,plasma NT-proBNP concentration was compared among different time points,as well as a-mong patients with different Glasgow Coma Scale(GCS)scores,among patients with different amounts of bleeding and among patients with different prognosis.Results Plasma NT-proBNP concentration in ob-servation group was higher than that in control group(P <0.01);Plasma NT-proBNP concentration at admission to hospital was higher than that at 7 days after admission(P<0.01);Plasma NT-proBNP con-centration in patients with GCS score≤8 was higher than that in patients with GCS score>8(P<0.01);Plasma NT-proBNP concentration increased with the amount of bleeding(P<0.01);Plasma NT-proBNP concentration in patients with improvement was lower than that in patients with death(P<0.01).Con-clusion Plasma NT-proBNP level significantly increases in patients with cerebral hemorrhage,especially in acute phase.Moreover,plasma NT-proBNP concentration is closely correlated with bleeding amount, neurological injury severity and prognosis.Therefore,the detection of plasma NT-proBNP has important significance in disease evaluation and prognosis prediction in patients with cerebral hemorrhage.
