目的:探讨达芬奇机器人手术系统在结直肠癌肝转移中的肝切除技术。方法回顾性分析我中心自2011年5月至2013年12月应用达芬奇机器人手术系统对4例结直肠癌肝转移患者行肝切除手术的临床资料。结果通过该系统,所有患者均成功完成手术。其中2例为单独切除肝转移瘤,2例为同期切除结肠原发灶及肝转移瘤。手术时间为210~510 min。术中失血量100~900 ml,输血1例。术后未发生并发症。术后住院时间8~15 d。结论达芬奇机器人手术系统能够安全有效地完成结直肠癌肝转移的微创外科手术。

목적:탐토체분기궤기인수술계통재결직장암간전이중적간절제기술。방법회고성분석아중심자2011년5월지2013년12월응용체분기궤기인수술계통대4례결직장암간전이환자행간절제수술적림상자료。결과통과해계통,소유환자균성공완성수술。기중2례위단독절제간전이류,2례위동기절제결장원발조급간전이류。수술시간위210~510 min。술중실혈량100~900 ml,수혈1례。술후미발생병발증。술후주원시간8~15 d。결론체분기궤기인수술계통능구안전유효지완성결직장암간전이적미창외과수술。
Objective To evaluate the technique of hepatectomy for metastatic colorectal cancer by Da Vinci surgical system. Methods Between May 2011 and Dec 2013, 4 patients underwent robotic liver resection for metastatic colorectal cancer at the PLA General Hospital and were analyzed retrospectively. Results The robotic surgeries were performed successfully on all the patients. Among the 4 surgeries, 2 patients underwent synchronous resection the colorectal cancer and liver metastatic tumor and 2 patients underwent hepatectomy of liver metastatic tumor. The operative time was 210~510 min.Intraoperative blood loss ranged from 100~900 ml and blood transfusions were required in 1 patients. The postoperative course was uneventful. The postoperative hospital stay was 8 ~15 days.Conclusions The minimally invasive surgery for hepatectomy for metastatic colorectal cancer was safe and feasible when assisted by the Da Vinci surgical system. Robotics offers a new technical option for minimally invasive hepatectomy for metastatic colorectal cancer.
