目的探讨鞘内注射右美托咪定( DEX)对坐骨神经慢性缩窄性损伤( CCI)大鼠脊髓背角肿瘤坏死因子α( TNF-α)蛋白表达的影响。方法成年Sprague-Dawley雄性大鼠44只随机分为四组( n=11):假手术组( sham组),坐骨神经慢性缩窄性损伤组( CCI组),生理盐水组( CCI+NS组)和右美托咪定组( CCI+DEX组)。采用von Frey纤毛分别测定了sham组和CCI组术前及术后1、3、5、7、14 d,以及CCI+NS组和CCI+DEX组术后第7天鞘内给药前和给药后1、2、3 h的机械刺激抬腿反应阈值( PWT)。 sham组和CCI组在术后第7天,CCI+NS组和CCI+DEX组在术后第7天鞘内给药后1 h时间点分别余5只大鼠处死取脊髓背角,采用Werstern blot法测定TNF-α蛋白表达。结果与sham组相比,CCI组PWT自术后第3天开始出现下降,并持续至第14天( P<0.001);与CCI+NS组相比,CCI+DEX组在给药后1 h即出现明显镇痛作用,且持续至给药后2 h(P<0.01或P<0.001)。 Werstern blot结果显示,和sham组比较,CCI组和CCI+NS组大鼠脊髓背角TNF-α蛋白表达上调( P<0.05);与CCI+NS组比较,CCI+DEX组TNF-α蛋白表达下调( P<0.05)。结论鞘内注射右美托咪定可通过抑制CCI大鼠脊髓背角TNF-α蛋白表达减轻神经病理性疼痛。

목적탐토초내주사우미탁미정( DEX)대좌골신경만성축착성손상( CCI)대서척수배각종류배사인자α( TNF-α)단백표체적영향。방법성년Sprague-Dawley웅성대서44지수궤분위사조( n=11):가수술조( sham조),좌골신경만성축착성손상조( CCI조),생리염수조( CCI+NS조)화우미탁미정조( CCI+DEX조)。채용von Frey섬모분별측정료sham조화CCI조술전급술후1、3、5、7、14 d,이급CCI+NS조화CCI+DEX조술후제7천초내급약전화급약후1、2、3 h적궤계자격태퇴반응역치( PWT)。 sham조화CCI조재술후제7천,CCI+NS조화CCI+DEX조재술후제7천초내급약후1 h시간점분별여5지대서처사취척수배각,채용Werstern blot법측정TNF-α단백표체。결과여sham조상비,CCI조PWT자술후제3천개시출현하강,병지속지제14천( P<0.001);여CCI+NS조상비,CCI+DEX조재급약후1 h즉출현명현진통작용,차지속지급약후2 h(P<0.01혹P<0.001)。 Werstern blot결과현시,화sham조비교,CCI조화CCI+NS조대서척수배각TNF-α단백표체상조( P<0.05);여CCI+NS조비교,CCI+DEX조TNF-α단백표체하조( P<0.05)。결론초내주사우미탁미정가통과억제CCI대서척수배각TNF-α단백표체감경신경병이성동통。
Objective To investigate the effect of intrathecal dexmedetomidine ( DEX ) on expression of tumor necrosis factor-alpha(TNF-α)in spinal dorsal horn of chronic constriction injury (CCI)rat.Methods Forty-four adult male Sprague-Dawley rats were randomly divided into 4 groups(n=11 each):sham operation group(group sham) ,CCI operation group ( group CCI ) , intrathecal normal saline ( NS ) in CCI rat group ( group CCI +NS ) , intrathecal dexmedetomidine in CCI rat group (group CCI+DEX).Paw withdrawal thresholds(PWTs)of group sham and group CCI were tested by Von Frey Filaments before and 1,3,5,7,14 day after operation.In addition,at 7 day after operation,paw withdrawal thresholds of group CCI +NS and group CCI +DEX were tested before and 1,2,3 hour after intrathecal normal saline/dexmedetomidine .The other 5 rats in each group were sacrificed at 7 day after operation in group sham and group CCI and 1 hour after intrathecal normal saline/dexmedetomidine in group CCI +NS and group CCI +DEX.The spinal dorsal horn were removed for determination of TNF-αprotein expression by Western blot .Results Compared to group sham ,the PWTs of group CCI were significantly decreased at 3 day after operation and continued to 14 day(P<0.001).After intrathecal dexmedetomidine ,the PWTs of group CCI +DEX were increased sharply compared with group CCI +NS ( P <0.01 or P <0.001 ) .Compared to group sham , the expression of TNF-αprotein was increased significantly in group CCI and group CCI +NS(P<0.05),whereas the expression level of TNF-αprotein in group CCI +DEX was lower than in group CCI +NS ( P <0.05 ) . Conclusions Intrathecal dexmedetomidine can reduce the mechanical allodynia through inhibiting the expression of TNF-αprotein in rat spinal dorsal horn .
