目的：探讨在特重度烧伤早期应用血液净化技术治疗的临床价值。方法：采用前瞻性随机对照研究，将我院38例特重度烧伤患者随机分为早期使用血液净化组和常规治疗组，每组各19例。2组患者均接受常规治疗，血液净化组在常规治疗基础上早期（48小时内）进行连续血液净化治疗。观察2组患者治疗前后TNF-α，IL-6，IL-10，血白细胞分类计数、动脉血气及生命体征的改变。结果：血液净化组患者治疗后血常规、动脉血气、TNF-α、IL-6、IL-10等数据较常规治疗组变化显著，差异有统计学意义（P＜0．05）。结论：特重烧伤患者在传统治疗方案基础上早期应用血液净化技术疗效显著，值得临床推广。
목적：탐토재특중도소상조기응용혈액정화기술치료적림상개치。방법：채용전첨성수궤대조연구，장아원38례특중도소상환자수궤분위조기사용혈액정화조화상규치료조，매조각19례。2조환자균접수상규치료，혈액정화조재상규치료기출상조기（48소시내）진행련속혈액정화치료。관찰2조환자치료전후TNF-α，IL-6，IL-10，혈백세포분류계수、동맥혈기급생명체정적개변。결과：혈액정화조환자치료후혈상규、동맥혈기、TNF-α、IL-6、IL-10등수거교상규치료조변화현저，차이유통계학의의（P＜0．05）。결론：특중소상환자재전통치료방안기출상조기응용혈액정화기술료효현저，치득림상추엄。
Objective:Evaluate clinical value of early use of blood purification in the treatment of severely burned .Methods:Clinical data about 38 cases with severely burned in the hospital were prospectively analyzed , which divided into 2 groups averagely ( contrast group and group of blood purification ) .All of the 38 cases had conventional treatments while the group of blood purification took the early blood purification (in 48 hours) on the basis of it.Observe TNF-α,IL-6 ,IL-10 ,WBC count ,Arterial blood gases and monitor vital signs dynamically .Results:Compare with contrast group , there are significant changes in all the criterions about group of blood purification . There are significant differences between groups (P<0.05).Conclusion:Early blood purification on the basis of traditional treatments is remarkable effect for severely burned patients .It is worth for clinical use .
