目的:分析研究心源性晕厥患者的动态心电图特征,为临床诊断提供依据。方法我院在2008年11月至2013年6月期间共收诊心源性晕厥患者43例,均行动态心电图检查,回顾性分析其临床资料。结果43例患者均发生严重的心律失常,其中15例(34.88%)、缓慢型心率失常28例(65.12%)。晕厥24例,发生率为55.81%。快速性心律失常导致晕厥的主要原因为超过200bpm的极快心室率,而缓慢性心律失常导致晕厥的主要原因为R-R间期平均值均大于3.0s。结论心源性晕厥与快速型、缓慢型心律失常有密切关系。动态心电图操作较简单、重复性好,可为临床诊断提供准确的病因依据,对心源性晕厥的诊断和治疗具有重要的临床意义。

목적:분석연구심원성훈궐환자적동태심전도특정,위림상진단제공의거。방법아원재2008년11월지2013년6월기간공수진심원성훈궐환자43례,균행동태심전도검사,회고성분석기림상자료。결과43례환자균발생엄중적심률실상,기중15례(34.88%)、완만형심솔실상28례(65.12%)。훈궐24례,발생솔위55.81%。쾌속성심률실상도치훈궐적주요원인위초과200bpm적겁쾌심실솔,이완만성심률실상도치훈궐적주요원인위R-R간기평균치균대우3.0s。결론심원성훈궐여쾌속형、완만형심률실상유밀절관계。동태심전도조작교간단、중복성호,가위림상진단제공준학적병인의거,대심원성훈궐적진단화치료구유중요적림상의의。
Objective To analyze the dynamic electrocardiogram characteristics of cardiac syncope,provide the basis for clinical diagnosis.Methods During Novem-ber 2008 to June 2013,there were 43 patients with cardiac syncope in our hospital,all were inspected by dynamic electrocardiogram,and the clinical data were retrospec-tively analyzed.Results 43 patients had occurred serious arrhythmia,the rapid arrhythmia in 15(34.88%),slow arrhythmia in 28 cases(65.12%).24 patients with syn-cope,the rate was 55.81%.The ventricular rate in rapid arrhythmia with syncope was more than 200 BPM.The R-R interphones in slow arrhythmia with syncope were more than a 3.0 s.Conclusion Cardiac syncope has a close relationship with fast,slow arrhythmia.Dynamic electrocardiogram(ecg)has the characteristic of relatively simple operation,good repeatability,it can provide the accurate basis of pathogen for clinical diagnosis ,and has important clinical significance on the diagnosis and treat-ment of central source sex syncope.
