目的探讨围手术期预防性抗生素应用受阻的情况。方法收集新疆医科大学第一附属医院2012年1-6月围手术期应用预防性抗菌药物医嘱总数9512例，采用新疆医科大学第一附属医院手术部设计的“预防性抗生素应用存在问题登记表”进行调查，数据分析采用SPSS17．0统计软件处理。结果9512例预防性抗菌素医嘱中应用受阻病例187例，其中延误6例，未执行181例。受阻原因依次为无术中医嘱或医嘱错误50例（26．74％），抗生素交接失误45例（24．06％），用药间隔时间太短38例（20．32％），皮试时间过期27例（14．44％），执行皮试医嘱不到位24例（12．83％），药物变质2例（1．07％），药物过敏1例（0．54％）。结论规范围手术期预防性抗生素用药流程，严格执行交接查对制度，加强护理人员药理知识和操作技能的培训，可减少抗生素应用受阻现象。

목적탐토위수술기예방성항생소응용수조적정황。방법수집신강의과대학제일부속의원2012년1-6월위수술기응용예방성항균약물의촉총수9512례，채용신강의과대학제일부속의원수술부설계적“예방성항생소응용존재문제등기표”진행조사，수거분석채용SPSS17．0통계연건처리。결과9512례예방성항균소의촉중응용수조병례187례，기중연오6례，미집행181례。수조원인의차위무술중의촉혹의촉착오50례（26．74％），항생소교접실오45례（24．06％），용약간격시간태단38례（20．32％），피시시간과기27례（14．44％），집행피시의촉불도위24례（12．83％），약물변질2례（1．07％），약물과민1례（0．54％）。결론규범위수술기예방성항생소용약류정，엄격집행교접사대제도，가강호리인원약리지식화조작기능적배훈，가감소항생소응용수조현상。
Objective To discuss the clinical application on preventive antibiotic during perioperative peri-od .Methods 9512 cases of doctor′s advice are collected on the clinical application on preventive antibacte-rial during perioperative period between January ,2012 and June ,2012 from Central Operating Room of the First Affliated Hospital of Xinjiang Medical University .The date was adopt the Prophylactic antibiotics application problems of the registration form design from Operating Room of the First Affliated Hospital of Xinjiang Medical University .The Data analysis was dealed with using SPSS17 .0 statistical software .Re-sults There were 187 cases of obstruction from the 9512 cases of doctor′s advice of prophylactic antibiot-ics .among which 187 cases ,6 cases of delayed ,181 cases were not performed .The reason were expounded and analyzed by 50 cases of absence or error of doctor′s advice during operation (26 .74% ) ,45 cases of misplays of antibiotic handover (24 .06% ) ,38 cases of time interval was too short (20 .32% ) ,27 cases of overdue skin tests before operation (14 .44% ) ,24 cases of nurses'improper disposal of doctor′s temporary advice (12 .83% ) ,2 cases of drug metamorphism (1 .07% ) ,one case of drug allergy (0 .53% ) .Conclu-sion Strict standardization throughout drug using process on preventive antibiotic during perioperative pe-riod ,strictly carry out handover check system and enhanced pharmacology training on nursing staff ,Anti-biotic application phenomenon were greatly reduced .
