目的探讨抗抑郁药物治疗对老年血管性痴呆患者执行功能的影响。方法将60例诊断为血管性痴呆（V D ）的患者随机分成治疗组与对照组，分别给予盐酸多奈哌齐及盐酸多奈哌齐＋抗抑郁药物治疗4 w。采用简易智能精神状态检查量表（MMSE）、执行性画钟作业（CLOX）及Stroop测验对两组治疗前后执行功能进行评定，对比两组治疗前后CLOX1、Stroop1、Stroop2评分。结果治疗组VD患者在服用盐酸多奈哌齐并加用SSRI类抗抑郁药物治疗4 w后，执行功能获得改善，与治疗前比较，差异有统计学意义（P ＜0．05）。对照组VD患者在单独服用盐酸多奈哌齐4 w后，执行功能有一定程度改善，但与治疗前比较差异无统计学意义（P ＞0．05）。而且治疗后，治疗组较对照组执行功能改善明显，差异具有统计学意义（P ＜0．05）。结论抗抑郁药物治疗可以改善老年VD患者的执行功能。
목적탐토항억욱약물치료대노년혈관성치태환자집행공능적영향。방법장60례진단위혈관성치태（V D ）적환자수궤분성치료조여대조조，분별급여염산다내고제급염산다내고제＋항억욱약물치료4 w。채용간역지능정신상태검사량표（MMSE）、집행성화종작업（CLOX）급Stroop측험대량조치료전후집행공능진행평정，대비량조치료전후CLOX1、Stroop1、Stroop2평분。결과치료조VD환자재복용염산다내고제병가용SSRI류항억욱약물치료4 w후，집행공능획득개선，여치료전비교，차이유통계학의의（P ＜0．05）。대조조VD환자재단독복용염산다내고제4 w후，집행공능유일정정도개선，단여치료전비교차이무통계학의의（P ＞0．05）。이차치료후，치료조교대조조집행공능개선명현，차이구유통계학의의（P ＜0．05）。결론항억욱약물치료가이개선노년VD환자적집행공능。
Objective To discuss the effect of antidepressant on executive function in old patients with vas-cular dementia .Methods Sixty patients diagnosed VD were divided into twe groups in randomization , treatment and control group .Treatment group was given donepezil hydrochloride and antidepressants . The control group was given donepezil hydrochloride only .Both for 4 weeks .All patients were evaluated with MMSE ,CLOX and Stroop test to assess the level of the executive function before and after the thera-py .Results The executive function of the treatment group improved after therapy of donepezil hydrochlo-ride and antidepressants for 4 weeks (P<0 .05) .The executive function of the control group also observed improved but not siginificant difference (P>0 .05) .After the therapy ,the treatment group had better ex-ecutive function than the control group obviously (P <0 .05) .Conclusion Antidepressants can improve the executive function in old patients with vascular dementia .
