目的探讨3.0T磁共振(MR)扩散张量成像(DTI)在脊髓型颈椎病早期诊断中的应用价值。方法选择20例健康志愿者作为对照组和80例T2WI无高信号的颈髓型颈椎病患者行颈髓常规磁共振成像(MRI)和DTI扫描,根据患者有无椎管狭窄将其分为椎管无狭窄组(A组,43例)和椎管狭窄组(B组,37例)。分析各组颈髓分数各向异性(FA)值和表观扩散系数(ADC)值的变化。结果对照组颈髓平均FA值为0.643±0.051,ADC值为(1.161±0.172)×10-3 mm2/s;A组和B组患者颈髓平均FA值分别为0.621±0.036、0.592±0.060;平均ADC值分别为(1.236±0.135)×10-3 mm2/s、(1.429±0.150)×10-3 mm2/s。三组之间两两比较,FA值对照组最高,A组次之,B组最低(P值均<0.05);ADC值对照组最低,A组次之,B组最高(P值均<0.05)。结论颈髓DTI成像较常规T2WI更早显示脊髓型颈椎病患者的颈髓损伤,椎管狭窄患者颈髓损伤程度更重。

Objective To evaluate the effects of DTI (Diffusion Tensor Imaging) in patients with CSM (Cervical Spondylotic Myelopathy). Methods Conventional MRI (Magnetic Resonance Imaging) and DTI were performed in 20 healthy volunteers who were selected as the Control group. While, 80 CSM sufferers without high signals to T2WI (T2-weighted Imaging) were scanned with conventional MRI and DTI by a 3.0T MR scanner and divided into two groups (group A:43 cases of patients without spinal stenoses; group B: 37 cases of patients with spinal stenoses) according to their scanning results. Then, analysis of the changes in FA (Fractional Anisotropy) and ADC (Apparent Diffusion Coefifcient) values were performed. Results The average FA and ADC values of the Control group were 0.643±0.051 and (1.161±0.172)×10-3mm2/s respectively. The FA values of group A and B were 0.621±0.036 and 0.592±0.060 respectively; the ADC values of group A and B were ( 1.236±0.135)×10-3mm2/s and (1.429±0.150)×10-3mm2/s respectively. On mean FA values, the Control Group ranked the ifrst while Group B the third ( p<0.05). As for the mean ADC values, group B was the highest ( p<0.05) while the Control group was the lowest. Conclusion In comparison with conventional T2WI, DTI allowed earlier manifestation of the spondylosis-related changes in CSM patients.
