目的比较原发干燥综合征(pSS)、系统性红斑狼疮(SLE)患者外周血浆中循环DNA(cDNA)浓度的差异性,探讨cDNA浓度在pSS、SLE诊治中的意义。方法选用人类基因组管家基因甘油醛-3-磷酸脱氢酶(GAPDH)设计引物,测定pSS、SLE及健康对照组中血浆cDNA浓度,同时测定血沉(ESR)、C-反应蛋白(CRP)、免疫球蛋白(IgA、IgG、IgM)并评定不同疾病的活动指数和损害指数,分析血浆cDNA浓度与疾病活动性、相关参数的关系。结果 pSS组cDNA浓度[(368.24±196.68)ng/ml]明显高于SLE组[(244.51±164.10)ng/ml]及健康对照组[(24.56±16.76)ng/ml],差异均有统计学意义(均P<0.05);pSS组中的cDNA浓度值与干燥综合征活动指数(SSDAI)呈正相关(P<0.05),与干燥综合征损害指数(SSDDI)无相关性(P>0.05);SLE组中cDNA浓度值与SLE活动指数(SLEDAI)无相关性(P>0.05)。结论 cDNA浓度可作为pSS与SLE鉴别指标之一,pSS患者cDNA浓度与SSDAI呈正相关,可作为pSS疾病活动性观察指标之一。

목적비교원발간조종합정(pSS)、계통성홍반랑창(SLE)환자외주혈장중순배DNA(cDNA)농도적차이성,탐토cDNA농도재pSS、SLE진치중적의의。방법선용인류기인조관가기인감유철-3-린산탈경매(GAPDH)설계인물,측정pSS、SLE급건강대조조중혈장cDNA농도,동시측정혈침(ESR)、C-반응단백(CRP)、면역구단백(IgA、IgG、IgM)병평정불동질병적활동지수화손해지수,분석혈장cDNA농도여질병활동성、상관삼수적관계。결과 pSS조cDNA농도[(368.24±196.68)ng/ml]명현고우SLE조[(244.51±164.10)ng/ml]급건강대조조[(24.56±16.76)ng/ml],차이균유통계학의의(균P<0.05);pSS조중적cDNA농도치여간조종합정활동지수(SSDAI)정정상관(P<0.05),여간조종합정손해지수(SSDDI)무상관성(P>0.05);SLE조중cDNA농도치여SLE활동지수(SLEDAI)무상관성(P>0.05)。결론 cDNA농도가작위pSS여SLE감별지표지일,pSS환자cDNA농도여SSDAI정정상관,가작위pSS질병활동성관찰지표지일。
Objective To investigate the plasma circulating DNA (cDNA)levels in patients with primary Sjogren's syn-drome(pSS) and systemic lupus erythematosus(SLE), and its clinical significance. Methods The primer was designed by human genome housekeeper gene GAPDH. Levels of cDNA, erythrocyte sedimentation rate (ESR), C- reaction protein (CRP) and im-munoglobulin (IgA, IgG and IgM) in each group were detected. Meanwhile, the activity index and damage index of different dis-eases were assessed, and the relations between cDNA and disease activity were analyzed. Results The levels of plasma cDNA in pSS group (368.24±196.68ng/ml) were apparently higher than those in SLE group (244.51±164.10ng/ml) and normal control group (24.56±16.76ng/ml),and the differences were significant statistical y (both P<0.05).The levels of cDNA in each disease group were significantly higher than those in normal control group (both P<0.05).In pSS group:cDNA level was positively corre-lation with Sjogren's syndrome disease activity index (SSDAI)(P<0.05).But there was no correlation between cDNA concentration and Sjogren's syndrome disease damage index (SSDDI)(P>0.05). In SLE group:cDNA level did not correlate with SLE disease activity index(SLEDAI)(P>0.05). Conclusion The cDNA level may be a valuable indicator for distinguishing pSS from SLE.And it can be taken as one of the disease activity indices of pSS.
