目的：采用生活质量量表（ EORTC QLQ-LC43）评价吉西他滨联合奥沙利铂每周方案治疗老年晚期非小细胞肺癌（NSCLC）的疗效。方法43例老年晚期NSCLC患者在给予支持及对症治疗的基础上，给予吉西他滨联合奥沙利铂每周方案，治疗2个周期后，评价近期疗效及毒副反应，并于治疗前及治疗2个周期后采用EORTC QLQ-LC43进行患者生活质量评价。结果43例共完成194个周期，临床客观有效率34.9％、疾病控制率76.7％。整体生活质量、5个功能量表、疲乏、恶心、呕吐及便秘症状量表评分均较治疗前升高（P＜0.05）；疼痛、呼吸困难及失眠症状量表评分较治疗前降低（P＜0.05）；食欲不振、腹泻及经济困难量表评分与治疗前比较无显著性差异（P＞0.05）。气促、咳嗽、咯血及胸痛等症状明显改善（P＜0.05）；脱发、手或肩部疼痛及其他部位疼痛较治疗前后评分比较无显著差异（ P＞0.05）；化疗后出现手足刺痛及口腔疼痛等症状。结论吉西他滨联合奥沙利铂每周方案能够改善老年晚期NSCLC患者的生活质量，是一个有效的治疗方案。
목적：채용생활질량량표（ EORTC QLQ-LC43）평개길서타빈연합오사리박매주방안치료노년만기비소세포폐암（NSCLC）적료효。방법43례노년만기NSCLC환자재급여지지급대증치료적기출상，급여길서타빈연합오사리박매주방안，치료2개주기후，평개근기료효급독부반응，병우치료전급치료2개주기후채용EORTC QLQ-LC43진행환자생활질량평개。결과43례공완성194개주기，림상객관유효솔34.9％、질병공제솔76.7％。정체생활질량、5개공능량표、피핍、악심、구토급편비증상량표평분균교치료전승고（P＜0.05）；동통、호흡곤난급실면증상량표평분교치료전강저（P＜0.05）；식욕불진、복사급경제곤난량표평분여치료전비교무현저성차이（P＞0.05）。기촉、해수、각혈급흉통등증상명현개선（P＜0.05）；탈발、수혹견부동통급기타부위동통교치료전후평분비교무현저차이（ P＞0.05）；화료후출현수족자통급구강동통등증상。결론길서타빈연합오사리박매주방안능구개선노년만기NSCLC환자적생활질량，시일개유효적치료방안。
Objective To assess the clinical efficacy of gemcitabine combined with oxaliplatin 1-week treatment on non -small cell lungcancer(NSCLC)intheagedbyquestionnaireoflifequality.Methods 48elderlypatientswithNSCLCweregiventhetherapyofox-aliplatin and gemcitabine 1-week on the basis of the support and symptomatic treatment.After 2 cycles of treatment , the recent curative effect, such as objective clinical response rate (ORR), disease control rate (DCR) and toxicity reactions were evaluated.Before and after treatment, the quality of life was evaluated by European Organization for Research and Treatment of Lung Cancer Quality of Life Question-naireLC-43(EORTCQLQ-LC43)too.Results 43patientscompleted194cyclestotally.ORRandDCRwere34.9%,76.7%respec-tively.After treatment, the scores of the physical function , roles function, emotional functions, social functions, cognitive function, general health state, fatigue, nausea/vomiting and constipation were higher than before (P<0.05).The scores of pain, dyspnea and insomnia were lower than before ( P<0.05 ).The scores of poor appetite , diarrhea and financial difficulty were similar as before.The symptoms of shortness of breath, cough, hemoptysis and chest pain were all improved.There were no significant differences between before treatment af-ter treatment about hair loss , hand or shoulder pain or other parts ( P>0.05 ).Hands and feet and oral pain appeared after chemothera-py.Conclusion Gemcitabine combined with oxaliplatin 1-week can improve the quality of life and is an effective therapy method for eld-erly patients with advanced NSCLC.
