目的:通过对特发性炎性肌病(IIM)合并间质性肺病(ILD)患者的血清表面活性蛋白A(SP-A)和D(SP-D)的研究,探讨其与IIM合并ILD的相关性。方法采用双抗体夹心酶联免疫吸附试验检测SP-A和SP-D,比较单纯IIM组、IIM-ILD组、对照组血清样本SP-A和SP-D水平的差异。分析IIM-ILD组SP-A和SP-D与年龄、肌力、红细胞沉降率(ESR)、C反应蛋白(CRP)及肌酶之间的相关性。结果①IIM-ILD组患者血清SP-A和SP-D水平高于单纯IIM组以及对照组(P<0.05)。单纯IIM组和健康对照组患者血清SP-A和SP-D水平比较差异无统计学意义(P>0.05)。血清SP-A、SP-D之间具有较高的正相关性。②IIM-ILD患者血清SP-A、SP-D水平与年龄、肌力、ESR、C反应蛋白及肌酶无相关性。结论 SP-A和SP-D与IIM合并ILD具有相关性,可作为IIM合并ILD的血清学标志,在早期诊断IIM合并ILD方面有一定价值。

목적:통과대특발성염성기병(IIM)합병간질성폐병(ILD)환자적혈청표면활성단백A(SP-A)화D(SP-D)적연구,탐토기여IIM합병ILD적상관성。방법채용쌍항체협심매련면역흡부시험검측SP-A화SP-D,비교단순IIM조、IIM-ILD조、대조조혈청양본SP-A화SP-D수평적차이。분석IIM-ILD조SP-A화SP-D여년령、기력、홍세포침강솔(ESR)、C반응단백(CRP)급기매지간적상관성。결과①IIM-ILD조환자혈청SP-A화SP-D수평고우단순IIM조이급대조조(P<0.05)。단순IIM조화건강대조조환자혈청SP-A화SP-D수평비교차이무통계학의의(P>0.05)。혈청SP-A、SP-D지간구유교고적정상관성。②IIM-ILD환자혈청SP-A、SP-D수평여년령、기력、ESR、C반응단백급기매무상관성。결론 SP-A화SP-D여IIM합병ILD구유상관성,가작위IIM합병ILD적혈청학표지,재조기진단IIM합병ILD방면유일정개치。
Objective To investigate the correlation between idiopathic inflammatory myopathy (IIM) and inter-stitial lung disease (ILD) by assessment of the levels of surfactant protein A (SP-A) and D (SP-D). Methods The lev-els of SP-A and SP-D in patients with IIM and those complicated with ILD (IIM-ILD) and healthy controls were as-sessed by using enzyme-linked immunosorbent assay. This allowed the comparison on the difference of SP-A and SP-D among the three groups and the correlation between SP-A and SP-D with age, muscular strength, erythrocyte sedimen-tation rate (ESR), C-reactive protein (CRP) and creatine kinase. Results The levels of SP-A and SP-D in patients with IIM-ILD were markedly higher than those in IIM group and the healthy control group (both P<0.05). The differ-ence in SP-A and SP-D was not statistically significant between the latter two groups (P>0.05). There was a significant positive correlation between SP-A and SP-D. The levels of SP-A and SP-D in patients with IIM-ILD were not correlat-ed with age, muscular strength, ESR, CRP and creatine kinase. Conclusion SP-A and SP-D are correlated with IIM-ILD and may be valuable for the early diagnosis of IIM-ILD.
