目的:探讨新生儿窒息血常规29项参数的变化及其临床意义。方法利用Sysmex XE-5000全自动血细胞分析仪对新生儿窒息40例(窒息组)和正常新生儿38例(对照组)进行白细胞、红细胞及血小板等参数检测,分析其结果和临床意义。结果(1)白细胞参数:窒息组中性粒细胞比率明显高于对照组(P<0.05),淋巴细胞比率、单核细胞比率明显低于对照组(P<0.05),白细胞计数、嗜酸性粒细胞比率、嗜碱性粒细胞比率比较,差异无统计学意义(P>0.05)。(2)红细胞参数:窒息组红细胞平均血红蛋白浓度、低荧光网织红细胞比率明显低于对照组( P<0.05),红细胞分布宽度CV、红细胞分布宽度SD、网织红细胞计数、网织红细胞百分比、未成熟网织红细胞百分比、高荧光网织红细胞比率、有核红细胞计数、有核红细胞百分比明显高于对照组( P<0.05);两组红细胞计数、血红蛋白、红细胞压积、红细胞平均体积、红细胞平均血红蛋白含量、中荧光网织红细胞比率、网织红细胞血红蛋白含量比较,差异无统计学意义(P>0.05)。(3)血小板参数:窒息组血小板计数、血小板压积明显低于对照组(P<0.05);两组平均血小板体积、血小板分布宽度、大型血小板比率及未成熟血小板指数比较,差异无统计学意义( P>0.05)。结论动态监测患儿血常规可以及时了解窒息患儿病情的变化,对新生儿窒息初步筛查、判定严重程度、早期干预及救治具有重要意义。

목적:탐토신생인질식혈상규29항삼수적변화급기림상의의。방법이용Sysmex XE-5000전자동혈세포분석의대신생인질식40례(질식조)화정상신생인38례(대조조)진행백세포、홍세포급혈소판등삼수검측,분석기결과화림상의의。결과(1)백세포삼수:질식조중성립세포비솔명현고우대조조(P<0.05),림파세포비솔、단핵세포비솔명현저우대조조(P<0.05),백세포계수、기산성립세포비솔、기감성립세포비솔비교,차이무통계학의의(P>0.05)。(2)홍세포삼수:질식조홍세포평균혈홍단백농도、저형광망직홍세포비솔명현저우대조조( P<0.05),홍세포분포관도CV、홍세포분포관도SD、망직홍세포계수、망직홍세포백분비、미성숙망직홍세포백분비、고형광망직홍세포비솔、유핵홍세포계수、유핵홍세포백분비명현고우대조조( P<0.05);량조홍세포계수、혈홍단백、홍세포압적、홍세포평균체적、홍세포평균혈홍단백함량、중형광망직홍세포비솔、망직홍세포혈홍단백함량비교,차이무통계학의의(P>0.05)。(3)혈소판삼수:질식조혈소판계수、혈소판압적명현저우대조조(P<0.05);량조평균혈소판체적、혈소판분포관도、대형혈소판비솔급미성숙혈소판지수비교,차이무통계학의의( P>0.05)。결론동태감측환인혈상규가이급시료해질식환인병정적변화,대신생인질식초보사사、판정엄중정도、조기간예급구치구유중요의의。
Objective To investigate the changes of the 29 parameters of blood routine in asphyxia newborns ,and to study the clinical significance .Methods The automatic blood cell analyzer Sysmex XE-5000 was used to analyze the parameters ,such as leukocytes ,erythrocytes and platelets in 40 asphyxia newborns ( asphyxia group ) and 38 normal newborns ( control group ) .The results and clinical significance were analyzed as well .Results ① The parameters of leukocytes:The neutrophil ratio(NEUT%) was significantly higher(P<0.05),the lymphocyte ratio(LYMPH%) and monocyte ratio ( MONO%) were significantly lower in the asphyxia group in contrast with those in the control group (P<0.05).The WBC count,eosinophil ratio (EO%) and basophils ratio (BASO%) showed no significant difference between two groups(P>0.05).②The parameters of erythrocytes:The mean hemoglobin concentration (MCHC) and low fluorescent reticulocyte percentage in the asphyxia group were significantly lower than those in the control group ( P<0.05) . The red blood cell distribution width CV(RDW-CV),red blood cell distribution width SD(RDW-SD),reticulocyte count (RET),percentage of reticulocytes(RET%),percentage of immature reticulocyte(IRF),high fluorescence reticulocyte ratio (HFR),nucleated red blood cell count (NRBC) and percentage of nucleated red blood cell (NRBC%) in the asphyxia group were significantly higher than those in the control group (P<0.05).No significant differences were found in the red blood cell count , hemoglobin, hematocrit , mean corpuscular volume ( MCV ) , content of mean corpuscular hemoglobin (MCH),ratio of median fluorescence reticulocytes(MFR) and content of hemoglobin in reticulocytes(RET-He)between two groups (P>0.05).③The parameters of platelets:The platelet count(PLT) and plateletcrit(PCT) in the asphyxia group were significantly lower than those in the control group (P<0.05).There were no significant differences in the mean platelet volume(MPV),platelet distribution width (PDW),platelet-large cell ratio (P-LCR) and immature platelet index (IPF) between two groups(P>0.05).Conclusion Dynamic monitoring of blood routine can help to learn the changes of asphyxia neonatorum.And it is of great significance to screening and diagnosis of the severity of neonatal asphyxia in time so as to intervene and rescue the patients in the early stage .
