目的:观察创伤性凝血病并发肺损伤的临床特点,寻找有效预防方法。方法40只日本大耳白兔经胸骨切开行剖胸探查术,制作创伤性凝血病动物模型,根据呼吸道有无出血,随机分为肺损伤组、非肺损伤组。记录简易动物呼吸机应用时间、呼吸机参数,评估呼吸机支持力度,测定血气分析,评估动物氧代谢情况,处死动物后测定肺组织局部IL-6水平,评估肺组织炎症情况。结果创伤性凝血病实验动物血气分析结果显示氧分压(PaO2),动脉血氧饱和度(SPO2)低下(P<0.05),肺泡-动脉氧分压差(P(A-a)O2)增高(P<0.05),呼吸机平均机械通气时间(6.26±0.82天),呼吸机辅助呼吸力度大(P<0.05),肺组织局部IL-6水平明显升高(P<0.05)。结论创伤性凝血病易于合并肺组织损伤,影响氧代谢,应引起临床重视。

Objective To observe the clinical characteristics of traumatic coagulopathy complicated with lung injury. Methods 40 Japanese rats were given exploratory thoracotomy through chest bone to set the model of traumatic coagulopathy. They were divided into two groups based on complicated lung injury or not. The duration of ventilator application, ventilator parameters and support were recorded. The blood gas analysis was evaluated and the level of IL-6 was detected. Results The results of blood gas analysis showed that the partial pressure of oxygen (PaO2), arterial oxygen saturation (SPO2) were lower in the injury group than in the control group (P<0. 05). The level of P( A-a) O2 increased, the average duration of mechanical ventilator was longer, and the level of IL-6 in-creased. Conclusion Traumatic coagulopathy is easy to have lung tissue with injury, oxygen metabolism, which should be paid more attention.
