目的:评估纤维支气管镜肺泡灌洗术对儿童重症肺炎支原体肺炎的治疗作用。方法对符合重症肺炎支原体肺炎诊断标准,同时符合大叶性肺炎诊断标准69例患儿,依据是否行支气管镜肺泡灌洗分为纤支镜组(35例)和对照组(34例),依据行肺泡灌洗前病程长短将纤支镜组分为10天内组(14例)和大于10天组(21例),比较各组实验室结果、临床特点、住院时间及炎症消散率。结果纤支镜组患儿热退时间、症状改善时间、肺部体征消失时间、住院时间及1月后肺部炎症消散率均优于对照组。10天内组患儿热退时间、症状改善时间、肺部体征消失时间及住院时间亦优于大于10天组。结论早期纤维支气管镜肺泡灌洗术可清除重症肺炎支原体肺炎气道痰栓,减轻炎症。

목적:평고섬유지기관경폐포관세술대인동중증폐염지원체폐염적치료작용。방법대부합중증폐염지원체폐염진단표준,동시부합대협성폐염진단표준69례환인,의거시부행지기관경폐포관세분위섬지경조(35례)화대조조(34례),의거행폐포관세전병정장단장섬지경조분위10천내조(14례)화대우10천조(21례),비교각조실험실결과、림상특점、주원시간급염증소산솔。결과섬지경조환인열퇴시간、증상개선시간、폐부체정소실시간、주원시간급1월후폐부염증소산솔균우우대조조。10천내조환인열퇴시간、증상개선시간、폐부체정소실시간급주원시간역우우대우10천조。결론조기섬유지기관경폐포관세술가청제중증폐염지원체폐염기도담전,감경염증。
Objective To evaluate the role of bronchoalveolar lavage with bronchofibroscope in the treatment of childhood with severe mycoplasma pnenmoniae pneumonia. Methods 69 patients who met severe mycoplasma pnenmoniae pneumonia standards and mycoplasma pnenmoniae lobar pneumonia diagnosis standards were divided into the fiber bronchofibroscope group (35 cases) and the control group (34 cases). According to the duration of disease clinical course, the fiber bronchofibroscope group was divided into the group of less than 10 days ( the group A, 14 cases) and the group of more than 10 days (the group B, 21cases). Laboratory results, clinical characteristics, du-ration of hospital stay and inflammation dissipation rate were documented. Results Compared to the control group, the improvement of defervescence, respiratory symptoms and signs, duration of hospital stay and infiltration absorption rate was more pronounced in the treatment group than in the control group 1 month after the treatment. Meanwhile, it was better improved in the group A than in the group B. Conclusion Bronchoalveolar lavage with bronchofibroscope in early course can remove airway sputum embolism, reduce inflammation of severe mycoplasma pnenmoniae pneumo-nia.
