目的:探讨固尔苏联合辅助通气治疗新生儿肺透明膜病的临床疗效。方法对我院新生儿科确诊为新生儿肺透明膜病的71例患儿随机分为观察组(37例)和对照组(34例),对照组采用常规治疗加辅助通气,观察组在对照组的治疗基础上联合气管内滴入肺表面活性物质(固尔苏)治疗,观察患儿治疗前后的临床表现、血气、X线胸片及存活率。结果固尔苏治疗组患儿使用固尔苏后30 min和12 h,PaO2显著升高, PaCO2明显下降,与对照组比较有差异,具有统计学意义(P<0.05)。观察组痊愈出院为67.6%,放弃治疗为10.8%,死亡21.6%;对照组痊愈出院44.1%,放弃治疗14.7%,死亡41.2%。结论固尔苏联合辅助通气治疗新生儿肺透明膜病能明显减轻患儿临床症状,改善患儿氧合功能,提高新生儿、特别是早产儿存活率,减少病死率。

Objective To discuss the therapeutic effect of Curosurf combined with assisted ventilation in the treatment of hyaline membrane disease. Methods 71 children, who were confirmed with hyaline membrane disease, were randomly divided into observation group (37 cases) and the control group (34 cases). The control group was given the conventional treatment with assisted ventilation. On the basis of the treatment in the control group, the ob-servation group was additionally given pulmonary surfactant ( Curosurf) . Their clinical manifestations, blood gas anal-ysis, chest X-rays and the survival rate were compared between the two groups. Results 30 minutes and 12 hours after the treatment, the level of PaO2 increased and the level of PaCO2 decreased more obviously in the observation group than in the control group (P<0. 05). The cure rate was 67. 6%, 10. 8% of them gave up the treatment, and the mortality was 21. 6% in the observation group. In the control group, it was 44. 1%, 17. 7% and 39. 3% respec-tively. Conclusion Curosurf combined with assisted ventilation can relieve the clinical symptoms and improve the oxygenations and survival rates in the treatment of hyaline membrane disease.
