目的：观察腹部手术病人术后布托啡诺、吗啡自控静脉镇痛（ PCIA ）对血清肿瘤坏死因子-α（TNF-α）、白细胞介素-6（IL-6）水平的影响。方法择期腹部手术患者30例，按随机数字表法分为布托啡诺组（A组）、吗啡组（B组）和对照组（C组），每组10例。术毕A组、B组行自控静脉镇痛（PCIA），C组在患者需求时给予肌注吗啡10 mg。3组分别于麻醉诱导前（T0）、术后12 h（T1）、24 h（T2）、36 h（T3）、48 h（T4）采集外周静脉血，采用酶联免疫吸附测定法测定血清中 TNF-α、IL-6水平。结果 A组、B组T1、T2、T3、T4各时点TNF-α和IL-6水平均低于C组（P＜0．05）。结论布托啡诺和吗啡PCIA能抑制术后患者血清TNF-α、IL-6水平的升高。

목적：관찰복부수술병인술후포탁배낙、마배자공정맥진통（ PCIA ）대혈청종류배사인자-α（TNF-α）、백세포개소-6（IL-6）수평적영향。방법택기복부수술환자30례，안수궤수자표법분위포탁배낙조（A조）、마배조（B조）화대조조（C조），매조10례。술필A조、B조행자공정맥진통（PCIA），C조재환자수구시급여기주마배10 mg。3조분별우마취유도전（T0）、술후12 h（T1）、24 h（T2）、36 h（T3）、48 h（T4）채집외주정맥혈，채용매련면역흡부측정법측정혈청중 TNF-α、IL-6수평。결과 A조、B조T1、T2、T3、T4각시점TNF-α화IL-6수평균저우C조（P＜0．05）。결론포탁배낙화마배PCIA능억제술후환자혈청TNF-α、IL-6수평적승고。
Objective To observe the impacts of patient-controlled intravenous analgesia (PCIA) with butorphanol and morphine on the serum levels of tumor necrosis factor α(TNF-α) and interleukin 6(IL-6) in the patients after abdominal operation.Methods Thirty patients undergoing selective abdominal operation were randomly divided into three groups:butorphanol group(group A),morphine group(group B) and control group(group C),with 10 cases in each group.The patients from group A and group B were given PCIA ,whereas patients from group C were intramuscularly injected with 10 mg of morphine when necessary .Peripheral venous blood samples were collected at 5 different time points of pre-anesthesia induction(T0),12 hours(T1),24 hours(T2),36 hours(T3) and 48 hours(T4) after operation. The levels of TNF-αand IL-6 in serum were measured by enzyme-linked immunosorbent assay ( ELISA ) .Results The levels of TNF-αand IL-6 at T1 , T2 ,T3 ,T4 of group A and group B were significantly lower than those of group C ( P<0.05) . Conclusion PCIA with butorphanol and morphine could effectively suppress the increase of serum levels of TNF-αand IL-6.
