目的：初步评价改良Hodge试验在产碳青霉烯酶肺炎克雷伯菌检测中的临床价值。方法用改良Hodge试验对170株超广谱β-内酰胺酶阳性肺炎克雷伯菌的碳青霉烯酶进行初步筛查,PCR扩增其基因并测序。结果在170株肺炎克雷伯菌中,10株对碳青霉烯类抗生素的敏感性降低,8株改良Hodge试验筛查阳性,经PCR证实8株均携带碳青霉烯酶-2型基因。结论改良Hodge试验能准确的筛查出临床分离株中存在的产碳青霉烯酶肺炎克雷伯菌,可以为临床治疗提供可靠的参考。

Objective To evaluate the application value of modified Hodge test ( MHT) in detecting Klebsiella pneumonia isolates producing KPC. Methods MHT was used to initially screen KPC from 170 strains of ESBLs pro-ducing Klebsiella pneumonia, then KPC were amplified to sequence by PCR. Results Of the 170 Klebsiella pneu-monia strains, there were 10 strains showing low sensitivity to carbapenems. 8 strains of them were positive by modi-fied Hodge test, and KPC-2 gene were detected in these 8 strains by PCR. Conclusion MHT can accurately detect the clinical isolates of pneumonia klebsiella producing KPC and provide reliable reference for clinical treatment.
