目的评价股骨近端防旋髓内钉（ PFNA）治疗高龄股骨转子间骨折的临床疗效。方法回顾性分析2009年6月至2013年3月28例在我院行股骨近端防旋髓内钉治疗的高龄股骨转子间骨折患者临床资料。结果所有病例均获随访，时间3～24个月，平均（12．5±1．5）个月。平均手术切口长度（5．5±1．3）cm，平均手术时间（50．3±3．6）min，平均术中出血量（175．6±25．4）ml，平均围手术期出血量（758．3±48．6）ml。术后无死亡病例，除1例因泌尿系统感染导致肺部感染转 ICU治愈，无其他并发症病例。骨折全部愈合，愈合时间11～16周，平均（12．9±1．4）周。按Harris评分进行评价，优20例，良6例，可2例，优良率92．9％。结论 PFNA具有手术创伤小、手术时间短、固定牢固的优点，是治疗高龄股骨转子间骨折安全有效的方法，但围手术期隐性失血量较显著，临床需密切关注。
목적평개고골근단방선수내정（ PFNA）치료고령고골전자간골절적림상료효。방법회고성분석2009년6월지2013년3월28례재아원행고골근단방선수내정치료적고령고골전자간골절환자림상자료。결과소유병례균획수방，시간3～24개월，평균（12．5±1．5）개월。평균수술절구장도（5．5±1．3）cm，평균수술시간（50．3±3．6）min，평균술중출혈량（175．6±25．4）ml，평균위수술기출혈량（758．3±48．6）ml。술후무사망병례，제1례인비뇨계통감염도치폐부감염전 ICU치유，무기타병발증병례。골절전부유합，유합시간11～16주，평균（12．9±1．4）주。안Harris평분진행평개，우20례，량6례，가2례，우량솔92．9％。결론 PFNA구유수술창상소、수술시간단、고정뢰고적우점，시치료고령고골전자간골절안전유효적방법，단위수술기은성실혈량교현저，림상수밀절관주。
Objective To analyze the effectiveness of PFNA for the treatment of femoral intertro-chanteric fractures in the aged.Methods A retrospective study was undertaken to analyze the clinical char-acteristics of 28 cases from June 2009 to March 2013 .Results All cases were followed up for 3-24 months,average(12.5 ±1.5)months.The mean length of incision was(5.5 ±1.3)cm ,the mean opera-tion time was(50.3 ±3.6)min,he mean intraoperative blood loss was(175.6 ±25.4)ml,and the calcu-lated mean total blood loss during operation was (758.3 ±48.6)ml.No one died,and there was no other complications except one case with pulmonary infection and was cured in ICU.Union occurred in all pa-tients with a mean healing time of(12.9 ±1.4)weeks.According to Harris score,20 cases obtained ex-cellent result,6 good result,2 fair result,and the excellent and good rate was 92 .9%.Conclusion PFNA is a very good method to treat intertrochanteric fractures in the aged,and there are some advantages of small trauma,less operation timeand rigid internal fixation.
