目的:探讨糖尿病足患者血流变与黏附因子变化并进行相关性分析。方法选择糖尿病足患者104例,根据Wagner分级分为1级、2级、3级,每组分别有36、36、32例。入选后分别行血流变及细胞黏附因子1(ICAM-1)、血管内皮细胞黏附因子1(VCAM-1)检测。结果2级患者低切黏度、血浆比黏度较1级患者有显著性升高,差异有统计学意义(P<0.05),3级患者低切黏度、血小板聚集指数、血浆比黏度较1级、2级患者有显著性升高,差异有统计学意义(P<0.05),高切黏度较1级患者相比差异有统计学意义(P<0.05)。2级患者 ICAM-1、VCAM-1较1级患者有显著性升高,差异有统计学意义(P<0.05),3级患者ICAM-1、VCAM-1较1级、2级患者有显著性升高,差异有统计学意义(P<0.05)。ICAM-1与低切黏度、血浆比黏度显著正相关性,差异有统计学意义(P<0.05),VCAM-1与低切黏度、血小板聚集指数、血浆比黏度比较差异有统计学意义(P<0.05)。结论糖尿病足患者存在显著的血流变与黏附因子异常,两者密切相关,共同参与糖尿病足的发生及发展。

목적:탐토당뇨병족환자혈류변여점부인자변화병진행상관성분석。방법선택당뇨병족환자104례,근거Wagner분급분위1급、2급、3급,매조분별유36、36、32례。입선후분별행혈류변급세포점부인자1(ICAM-1)、혈관내피세포점부인자1(VCAM-1)검측。결과2급환자저절점도、혈장비점도교1급환자유현저성승고,차이유통계학의의(P<0.05),3급환자저절점도、혈소판취집지수、혈장비점도교1급、2급환자유현저성승고,차이유통계학의의(P<0.05),고절점도교1급환자상비차이유통계학의의(P<0.05)。2급환자 ICAM-1、VCAM-1교1급환자유현저성승고,차이유통계학의의(P<0.05),3급환자ICAM-1、VCAM-1교1급、2급환자유현저성승고,차이유통계학의의(P<0.05)。ICAM-1여저절점도、혈장비점도현저정상관성,차이유통계학의의(P<0.05),VCAM-1여저절점도、혈소판취집지수、혈장비점도비교차이유통계학의의(P<0.05)。결론당뇨병족환자존재현저적혈류변여점부인자이상,량자밀절상관,공동삼여당뇨병족적발생급발전。
Objective To investigate the significance of changes in blood flow and adhesion molecules in pa-tients with diabetic foot .Methods 104 patients with diabetic foot according to Wagner classification were divided in-to grade one ,two ,three ,which had respectively 36 ,36 ,32 cases .Blood flow changes and ICAM-1 ,VCAM-1 were de-tected .Results Low shear viscosity ,plasma were significantly increased in grade two patients compared to grade 1 patient (P<0 .05) ,low shear viscosity ,platelet aggregation index ,plasma viscosity of grade 3 compared with grade one ,two patients significant increased (P<0 .05) ,compared with high shear viscosity of grade 1 patients showed sig-nificant difference (P<0 .05) .ICAM-1 ,VCAM-1 of grade 2 patients compared with grade 1patients significantly in-creased (P<0 .05) ,ICAM-1 ,VCAM-1 of grade 3 compared with grade 1 ,2 patients were significantly higher (P<0 .05) .ICAM-1 and low shear viscosity ,plasma viscosity showed significant positive correlation (P<0 .05) ,VCAM-1 and low shear viscosity ,platelet aggregation index ,plasma viscosity was significantly associated (P<0 .05) .Conclu-sion Patients with diabetic foot showed significant changes in blood flow abnormalities and adhesion molecules , which are closely related to participate in the occurrence and development of diabetic foot .
