目的：评估对比配对的胃腺癌活检标本及外科病理手术标本临床病理学特征及HER2表达，研究其异质性并评估临床意义。方法：选取2011年1月-2013年1月本院诊断的胃食管交界区普通型腺癌内镜活检标本及相应的肿瘤手术切除标本患者98例，并收集相应的临床病理学资料，用免疫组化（IHC）及原位荧光杂交（FISH）法检测内镜活检标本及相应的肿瘤手术切除标本HER2蛋白过表达和基因扩增情况，观察并分析其异质性表达的情况，分析临床病理学特征，评估临床意义。结果：98例胃腺癌患者中免疫组化HER2阳性16例（16.3%），FISH HER2阳性14例（14.3%），12例（75%）免疫组化HER2阳性胃腺癌组织中HER2呈现明显异质性，而FISH检测HER2阳性胃腺癌组织中仅有5例（41.7%）。结论：HER2在胃癌组织中呈现明显的异质性；活检标本能良好地预测胃癌手术标本HER2表达，应该对活检标本进行HER2检测。

목적：평고대비배대적위선암활검표본급외과병리수술표본림상병이학특정급HER2표체，연구기이질성병평고림상의의。방법：선취2011년1월-2013년1월본원진단적위식관교계구보통형선암내경활검표본급상응적종류수술절제표본환자98례，병수집상응적림상병이학자료，용면역조화（IHC）급원위형광잡교（FISH）법검측내경활검표본급상응적종류수술절제표본HER2단백과표체화기인확증정황，관찰병분석기이질성표체적정황，분석림상병이학특정，평고림상의의。결과：98례위선암환자중면역조화HER2양성16례（16.3%），FISH HER2양성14례（14.3%），12례（75%）면역조화HER2양성위선암조직중HER2정현명현이질성，이FISH검측HER2양성위선암조직중부유5례（41.7%）。결론：HER2재위암조직중정현명현적이질성；활검표본능량호지예측위암수술표본HER2표체，응해대활검표본진행HER2검측。
To evaluate contrast match biopsy specimens of gastric adenocarcinoma demonstrate mitoses and surgical pathology specimen clinical pathology characteristic and the expression of HER2,study its heterogeneity and to assess clinical significance.Method:98 patients who were diagnosed adenocarcinoma of the stomach esophagus border area type endoscopic biopsy specimens and the corresponding tumor patients with surgically resected specimens in our hospital from January 2011 to January 2013 were selected,and collected the corresponding clinical pathology data. The endoscopic biopsy specimens and the corresponding tumor resection specimens HER2 protein expression and gene amplification were detected by immunohistochemical(IHC)and fluorescence in situ hybridization(FISH)method,the expression of heterogeneity was observed and analyzed,the characteristic of clinical pathology was analyzed and the clinical significance was evaluated.Result:98 patients with gastric adenocarcinoma immunohistochemical HER2 positive were 16 cases(16.3%),FISH HER2 positive were 14 cases(14.3%),12 cases(75%)immunohistochemical HER2 positive HER2 in gastric adenocarcinoma tissue presents obvious heterogeneity,and FISH to detect HER2 positive gastric adenocarcinoma tissues was only 5 cases(41.7%).Conclusion:The HER2 present obvious heterogeneity in gastric cancer tissues,biopsy specimen can well predict surgical specimens of gastric cancer HER2 expression,should HER2 detection on biopsy specimens.
