目的:探讨有创颅内压监测技术在重型颅脑损伤患者中的临床应用价值。方法:选取本科2012年7月-2013年12月应用有创颅压监测技术的114例重型颅脑损伤患者进行前瞻性研究,按照随机数字表法将其分为监测组和对照组各57例。监测组于入院后24 h内或开颅手术后行有创颅内压监测;对照组依据患者意识、生命体征和CT检查等,进行常规治疗。对比两组患者治疗3个月后GOS评分。结果:监测组的预后良好率为66.7%明显高于对照组的45.6%,差异有统计学意义(P<0.05)。结论:有创颅内压监测技术能动态观察重型颅脑损伤患者的颅内压变化情况,有利于及时采取有效治疗措施控制颅内压,改善患者预后,值得临床推广应用。

목적:탐토유창로내압감측기술재중형로뇌손상환자중적림상응용개치。방법:선취본과2012년7월-2013년12월응용유창로압감측기술적114례중형로뇌손상환자진행전첨성연구,안조수궤수자표법장기분위감측조화대조조각57례。감측조우입원후24 h내혹개로수술후행유창로내압감측;대조조의거환자의식、생명체정화CT검사등,진행상규치료。대비량조환자치료3개월후GOS평분。결과:감측조적예후량호솔위66.7%명현고우대조조적45.6%,차이유통계학의의(P<0.05)。결론:유창로내압감측기술능동태관찰중형로뇌손상환자적로내압변화정황,유리우급시채취유효치료조시공제로내압,개선환자예후,치득림상추엄응용。
To investigate the clinical application value of invasive intracranial pressure(ICP) monitoring in treatment of severe traumatic brain injury(sTBI).Method:A total of 114 patients with sTBI who were used invasive intracranial pressure monitoring in our hospital from July 2012 to December 2013 were selected for prospective studies,they were randomly divided into the monitoring group and the control group,57 cases in each group. The monitoring group was treated with invasive ICP monitoring within 24 h after admission or after craniotomy operation. While the control group was performed by routine treatment according to the patient’s consciousness,vital signs and CT examination,ect.The GOS score after 3 months of treatment between the two groups was compared.Result:The favorable prognosis rate of monitoring group was 66.7%,while it was significantly higher than 45.6%of the control group,the difference was statistically significant(P<0.05).Conclusion:During the treatment of sTBI patients,the invasive intracranial pressure monitoring can dynamicly observe ICP,and improve the prognosis of sTBI by controlling the ICP effectively,is worthy of clinical application.
