目的：探讨运动干预及社区管理对2型糖尿病患者的影响。方法选择北京市西城区德胜社区卫生服务中心综合内科病房2011-2012年收治的2型糖尿病患者100例，按患者入院时间顺序编号，采用随机数字表法将患者分为社区管理组和运动干预组，每组50例。社区管理组患者进行社区管理，运动干预组患者在社区管理组基础上进行6个月的运动干预。分别于入院时、干预6个月后测定两组患者空腹血糖（FPG）、餐后2 h 血糖（2 hPG）、体质指数（BMI）、糖化血红蛋白（HbA1c ）、三酰甘油（TG）、总胆固醇（TC）、高密度脂蛋白胆固醇（HDL - C）、低密度脂蛋白胆固醇（LDL - C）。结果入院时，两组患者 FPG、2 hPG、BMI、HbA1c、TG、TC、LDL - C比较，差异均无统计学意义（P >0.05）；社区管理组患者HDL - C高于运动干预组，差异有统计学意义（P é0.05）。干预6个月后，运动干预组患者 FPG、2 hPG、HbA1c、TG、TC、LDL - C低于社区管理组，HDL - C高于社区管理组，差异有统计学意义（P é0.05）；两组患者 BMI 比较，差异无统计学意义（P >0.05）。干预6个月后，社区管理组、运动干预组患者 FPG、2 hPG、BMI、HbA1c、TG、TC、LDL - C均较入院时降低，HDL - C较入院时升高，差异有统计学意义（P é0.05）。结论社区管理和运动干预在2型糖尿病患者的治疗中均起着重要作用，在社区管理的基础上进行运动干预有助于其血糖控制，还能有效地调节其血脂；开设社区卫生服务中心病房在慢性病的规范化管理中具有重要意义。
목적：탐토운동간예급사구관리대2형당뇨병환자적영향。방법선택북경시서성구덕성사구위생복무중심종합내과병방2011-2012년수치적2형당뇨병환자100례，안환자입원시간순서편호，채용수궤수자표법장환자분위사구관리조화운동간예조，매조50례。사구관리조환자진행사구관리，운동간예조환자재사구관리조기출상진행6개월적운동간예。분별우입원시、간예6개월후측정량조환자공복혈당（FPG）、찬후2 h 혈당（2 hPG）、체질지수（BMI）、당화혈홍단백（HbA1c ）、삼선감유（TG）、총담고순（TC）、고밀도지단백담고순（HDL - C）、저밀도지단백담고순（LDL - C）。결과입원시，량조환자 FPG、2 hPG、BMI、HbA1c、TG、TC、LDL - C비교，차이균무통계학의의（P >0.05）；사구관리조환자HDL - C고우운동간예조，차이유통계학의의（P é0.05）。간예6개월후，운동간예조환자 FPG、2 hPG、HbA1c、TG、TC、LDL - C저우사구관리조，HDL - C고우사구관리조，차이유통계학의의（P é0.05）；량조환자 BMI 비교，차이무통계학의의（P >0.05）。간예6개월후，사구관리조、운동간예조환자 FPG、2 hPG、BMI、HbA1c、TG、TC、LDL - C균교입원시강저，HDL - C교입원시승고，차이유통계학의의（P é0.05）。결론사구관리화운동간예재2형당뇨병환자적치료중균기착중요작용，재사구관리적기출상진행운동간예유조우기혈당공제，환능유효지조절기혈지；개설사구위생복무중심병방재만성병적규범화관리중구유중요의의。
Objective To observe the effect of eXercise intervention and community management on type 2 diabe-tes. Methods Between 2011 and 2012,100 hospitalized patients with type 2 diabetes admitted to the Comprehensive Internal Wards of Desheng Community Health Serice Center,Xicheng District,Beijing,were selected and according to the admission time number,were randomly assigned to the community management group who only received community management and eXer-cise intervention group(each group 50 patients) who received additional 6 - month eXercise intervention besides community management. Fasting plasma glucose(FPG)and 2 h postprandial blood glucose(2 hPG),body mass indeX(BMI),glycosy-lated hemoglobin(HbA1c),triglyceride(TG),total cholesterol(TC),higher density lipoprotein cholesterol(HDL- C), lower density lipoprotein cholesterol(LDL-C)of the two groups were measured on admission and after 6 - month of interven-tion. Results On admission,there was no statistic differences in FPG,2 hPG,BMI,HbA1c ,TG,TC and LDL - C between the two groups(P>0.05);HDL - C of the patients in community management group was higher than in eXercise intervention group and the difference was significant( P<0.05). After 6 - month of intervention,FPG,2 hPG,HbA1c ,TG,TC and LDL - C in eXercise intervention group were higher than in community management group eXcept HDL - C that was lower and the differences were significant(P<0.05);there was no significant difference of BMI between the two groups(P>0.05). FPG, 2 hPG,BMI,HbA1c ,TG,TC and LDL - C of the two groups after 6 - month of intervention were all lower than those on admis-sion eXcept HDL-C that was higher(P<0.05). Conclusion FXercise intervention and community management all play an important role in the treatment of type 2 diabetes. EXercise intervention combined with community management can not only control blood sugar but also effectively regulate blood lipid. The establishment of community health service center ward is very significant for the standardized management of the treatment of the chronic diseases.
