目的:探讨翼状胬肉切除联合角膜干细胞移植术后使用糖皮质激素滴眼液最佳持续时间。方法2009年1月~2011年6月于我院行翼状胬肉切除联合角膜干细胞移植术的328例患者(均为单眼手术)随机分为A(110例),B(110例),C(108例)三组。三组患者术后待角膜上皮修复后,均使用妥布霉素地塞米松滴眼液滴眼,3次/d。其中A组持续使用1w,B组持续使用2w,C组持续使用3w。术后随访2年,比较三组患者术后胬肉复发率和激素性青光眼发生率。结果A组胬肉复发率为19.09%,激素性青光眼发生率为0.91%。B组胬肉复发率为6.36%,激素性青光眼发生率为1.82%。C组胬肉复发率为5.56%,激素性青光眼发生率为8.33%。AB两组比较,胬肉复发率差异有统计学意义(χ2=8.0208,<0.01),激素性青光眼发生率差异无统计学意义(χ2=0.0000,>0.05);BC两组比较,胬肉复发率差异无统计学意义(χ2=0.0818,>0.05),激素性青光眼发生率有差异有统计学意义(χ2=4.8279,<0.05)。结论翼状胬肉切除联合角膜干细胞移植术后使用糖皮质激素最佳持续时间为2w左右。

목적:탐토익상노육절제연합각막간세포이식술후사용당피질격소적안액최가지속시간。방법2009년1월~2011년6월우아원행익상노육절제연합각막간세포이식술적328례환자(균위단안수술)수궤분위A(110례),B(110례),C(108례)삼조。삼조환자술후대각막상피수복후,균사용타포매소지새미송적안액적안,3차/d。기중A조지속사용1w,B조지속사용2w,C조지속사용3w。술후수방2년,비교삼조환자술후노육복발솔화격소성청광안발생솔。결과A조노육복발솔위19.09%,격소성청광안발생솔위0.91%。B조노육복발솔위6.36%,격소성청광안발생솔위1.82%。C조노육복발솔위5.56%,격소성청광안발생솔위8.33%。AB량조비교,노육복발솔차이유통계학의의(χ2=8.0208,<0.01),격소성청광안발생솔차이무통계학의의(χ2=0.0000,>0.05);BC량조비교,노육복발솔차이무통계학의의(χ2=0.0818,>0.05),격소성청광안발생솔유차이유통계학의의(χ2=4.8279,<0.05)。결론익상노육절제연합각막간세포이식술후사용당피질격소최가지속시간위2w좌우。
Objective To investigate the optimal continual time for applying glucorticoid eye drops after pterygium resection with autologous corneal limbal stem celltransplantation.Methods Patients (n=328) who underwent pterygium resection with autologous corneal limbal stem celltransplantation from January 2009 and June 2011 in Daye City People,s Hospital were randomly divided into 3 groups:group A(n=110)、group B (n=110) and group C(n=108).After surgeries,al patients were treated with tobramycin/dexamethasone eye drops after corneal epithelium healed .The continual time for applying tobramycin/dexamethasone eye drops were 1week,2 weeks and 3 weeks in group A,B and C, respectively. All patients received a postoperative 2-year fol ow up. The recurrent rate of pterygium and the incidence rate of glucocorticoid -induced glaucoma were compared among 3 groups.Results Group A:The recurrent rate of pterygium was 19.09%,the incidence rate of glucocorticoid-induced glaucoma was 0.91%.Group B:The recurrent rate of pterygium was 6.36%,the incidence rate of glucocorticoid-induced glaucoma was 1.82%. Group C:The recurrent rate of pterygium was 5.56%,the incidence rate of glucocorticoid-induced glaucoma was 8.33%. Between group A and B:there were statistical dif erences in the recurrent rate of pterygium(χ2=8.0208,<0.01).there were no statistical dif erences in the incidence rate of glucocorticoid-induced glaucoma(χ2=0.0000,>0.05);Between group B and C:There were no statistical dif erences in the recurrent rate of pterygiu (χ2=0.0818,>0.05),there were statistical dif erences in the incidence rate of glucocorticoid-induced glaucoma(χ2=4.8279,<0.05).Conclusion The optimal continual time for applying glucorticoid eye drops after pterygium resection with autologous corneal limbal stem celltransplantation was about 2 weeks.
