目的:探讨顽固性鼻出血的治疗方法。方法对我院2011年9月至2013年10月收治的52例顽固性鼻出血患者行鼻内镜检查,并对止血效果进行顾性总结分析。结果通过鼻内镜引导寻找到出血点,发现常见出血部位以鼻中隔嗅裂区、鼻中隔后端为主,根据出血的部位和出血的程度,对患者采用微波烧灼、射频止血、局部止血膨胀海绵填塞等不同的止血方法,于术后3~6天出院,患者均1次治愈。无鼻腔粘连、鼻中隔穿孔及心脑血管意外等并发症出现。结论治疗顽固性鼻出血的关键是在鼻内镜下准确找到出血点,针对性强、止血准确迅速、反应轻、痛苦小、安全,值得推广应用。

목적:탐토완고성비출혈적치료방법。방법대아원2011년9월지2013년10월수치적52례완고성비출혈환자행비내경검사,병대지혈효과진행고성총결분석。결과통과비내경인도심조도출혈점,발현상견출혈부위이비중격후렬구、비중격후단위주,근거출혈적부위화출혈적정도,대환자채용미파소작、사빈지혈、국부지혈팽창해면전새등불동적지혈방법,우술후3~6천출원,환자균1차치유。무비강점련、비중격천공급심뇌혈관의외등병발증출현。결론치료완고성비출혈적관건시재비내경하준학조도출혈점,침대성강、지혈준학신속、반응경、통고소、안전,치득추엄응용。
Objective to investigate the treatment of intractable epistaxis. Methods 52 cases of intractable epistaxis underwent endoscopic examination from 2011 september to 2013 october in our hospital were selected and the hemostatic effects were retrospectively analyzed. Results the bleeding points were found by the endoscopic guide. We found that common bleeding sites was in the the regions of olfactory cleft nasal septum and maily in the back of nasal septum. according to the site and the degree of bleeding, patients were treated with different methods including microwave cauterization, rf hemostasis or local hemostasis swelling sponge packing hemostasis. and patients often discharged in 3 - 6 days and only need one time for cure. also, there were no nasal cavity adhesion, perforation of nasal septum and cardio cerebral vascular accident. Conclusion Accurately finding the bleeding sites in endoscopic hemostasis is the key for the treatment of intractable epistaxis. It has the advantages of strong pertinence, quick and accurate hemostasis, light adverse reaction, less pain and safety. it is worthy of popularization and application.
