目的:调查高年资护士从事老年长期照护服务的意愿及影响因素。方法采取分层抽样方法,于2013年1-5月选取河南省15所医院的高年资护士384名,调查一般资料,高年资护士从事老年长期照护服务意愿、临床经历及认知,健康自评,老年人态度量表( KAOP)评分、老化知识量表( FAQ)评分、职业认同量表评分。采用二分类Logistic回归分析高年资护士从事老年长期照护服务意愿的影响因素。结果384名高年资护士中126名(32.8%)愿意从事老年长期照护服务。高年资护士KAOP评分(144.4±13.3)分,FAQ评分(10.8±3.5)分,职业认同量表评分(26.7±8.4)分。愿意和不愿意从事老年长期照护服务高年资护士KAOP评分、FAQ评分、职业认同量表评分比较,差异均有统计学意义( P<0.05)。不同最高学历、健康自评、福利待遇、是否忙于照料家庭、是否有老年病房工作经验、是否接受长期照护相关培训、是否关注老龄化相关内容的高年资护士从事老年长期照护服务意愿不同,差异均有统计学意义( P<0.05)。二分类Logistic回归分析结果显示,KAOP评分、FAQ评分、健康自评、福利待遇、是否有老年病房工作经验、是否接受长期照护相关培训、是否关注老龄化相关内容与高年资护士从事老年长期照护服务意愿有回归关系( P<0.05)。结论高龄、空巢、复合慢性病和失能失智老年人是迫切需要长期专业照护服务的群体,该领域也是展示护理专业价值的重要平台。针对不愿从事此项工作的深层次原因进行持续跟踪和多元分析,通过理顺管理、开展培训、提高福利、资源共享等,吸引和鼓励高年资护士加入老年长期照护服务行列,延伸服务领域,减少优质资源的闲置和浪费,弥补医养结合机构专业人员不足,减轻家庭和社会负担。

Objective To investigate the willingness and influencing factors for senior nurses involving in long term care for the elderly. Methods Stratified sampling was used to select 384 senior nurses from 15 hospitals in Henan province from January to May in 2013. General information,willingness,clinical experience and cognition of senior nurses about involving in long term care and self-rated health were investigated,and the score of Koganˊs attitudes towards old people( KOPA),facts on aging quiz( FAQ)and profession identification scale were utilized in the investigation. Binary Logistic analysis was adopted to an-alyze the influencing factors. Results 126 of the 384 senior nurses(32. 8%)were willing to engage in long term care of the eld-erly. The scores of KOPA,FAQ and profession identification scale were(144. 4 ± 13. 3),(10. 8 ± 3. 5),(26. 7 ± 8. 4),re-spectively. The scores of KOPA,FAQ and profession identification scale between senior nurses who were willing to engage in long term care of the elderly and those who were not showed statistically significant differences(P<0. 05). Senior nurses with differ-ent degree,self-rate health,social benefits,whether being busy taking care of family,having the experience working in geriatric unit,getting training about long term care and whether paying attention to population aging showed statistically significant differ-ences in the willingness of engaging in long term care of the elderly(P<0. 05). Binary Logistic analysis showed that the scores of KOPA and FAQ,self-rate health,social benefits,having the experience working in geriatric unit,getting training about long term care and whether paying attention to population aging were the influencing factors for the willingness to engage in long term care of the elderly(P<0. 05). Conclusion Senior citizens,empty nesters,the elderly with multiple chronic diseases and the disabled old person are the groups in sore need of long term care. The realm is a significant platform for showing the professional value of nursing. The aim is to keep track and conduct multiple analysis directing at the deep reasons of unwillingness to engage in long term care,and to attract senior nurses to join in long term care through straightening management,carrying out training,im-proving benefits and sharing resources in order to extend services and decrease the idleness and waste of high quality human re-source,as well as to make up the shortage of professional staff and relieve the burden both family and society.
