目的:了解2011年石家庄市体检人群超重、肥胖流行特征,并探讨黑素皮质素4受体( MC4R)基因rs17782313位点多态性与肥胖的关系。方法选取2011年河北省人民医院体检中心48831例18岁以上体检者的体检资料,对其超重、肥胖现状及影响因素进行流行病学分析。应用直接测序法对其中的3000例肥胖者(肥胖组)和1000例正常体质量者(对照组) MC4R基因rs17782313位点多态性进行分析,比较两组的基因型和等位基因频率分布情况。结果48831例体检者中,体质指数(BMI)平均(24.7±5.1)kg/m2,腰围平均(84.3±32.5)cm,超重、肥胖检出率分别为37.98%(18547/48831)、16.62%(8117/48831)。男性超重、肥胖检出率〔45.05%(12513/27775)和21.78%(6049/27775)〕高于女性〔28.66%(6034/21056)和9.82%(2068/21056)〕(P<0.05)。无论是男性还是女性,不同年龄的体检人群中超重、肥胖检出率间有差异( P <0.05)。肥胖组患者 MC4R 基因rs17782313位点的基因型、等位基因的频率分布与对照组比较有差异( P <0.05)。多因素 Logistic 回归分析显示, MC4R CC基因型是肥胖的独立危险因素〔OR=2.600,95%CI(1.418,4.764)〕。结论石家庄市体检人群超重和肥胖检出率高;MC4R基因rs17782313位点多态性与肥胖有关。

Objective To analyze the current situation of obesity and overweight in Shijiazhuang check-up crowds and its relationship with rs17782313 site polymorphism of MC4R gene. Methods Data of 48 831 cases over 18 years admitted to the Physical Examination Center of Peopleˊs Hospital of Hebei Province in 2011 were collected. The condition and risk factors of o-besity and overweight were analyzed in terms of epidemiology. Direct sequencing method was applied to test the rs17782313 site polymorphism of MC4R gene in obesity group(3 000 cases)and control group(1 000 cases with normal BMI),and the geno-type and allele frequency were compared between the two groups. Results Among the 48 831 cases,the average BMI was(24. 7 ±5. 1)kg/m2,the average WC was(84. 3 ± 32. 5)cm,and the prevalence rate of overweight and obesity was 37. 98%(18 547/48 831)and 16. 62% ( 8 117/48 831 ) . The prevalence rate of overweight and obesity for male was 45. 05%(12 513/27 775)and 21. 78%(6 049/27 775),higher than those of the female,which was 28. 66%(6 034/21 056)and 9. 82%(2 068/21 056)respectively(P <0. 05). The genotype and frequency distribution of allele of rs17782313 site of MC4R gene in obesity group showed statistically significant difference compared with the control group(P<0. 05). Multiple Lo-gistic regression analysis showed that MC4R genotype CC was the independent risk factor for obesity〔OR =2. 600,95%CI (1. 418,4. 764)〕. Conclusion Both prevalence rate of overweight and obesity are high in Shijiazhuang check-up crowds, and the rs17782313 site polymorphism of MC4R gene may be the pathogenic factor of obesity and overweight.
