目的:探讨小儿神经母细胞瘤化疗后严重骨髓抑制的临床表现及治疗。方法分析山西省儿童医院从2012年1月至2013年1月收治的12例小儿神经母细胞瘤化疗所引起的严重骨髓抑制情况。结果12例患儿均发生Ⅳ期骨髓抑制,其中3例在交替化疗过程中发生2次骨髓抑制,均无严重并发症,经治疗2周内骨髓造血功能恢复正常。结论小儿神经母细胞瘤化疗引起的骨髓抑制,经早期发现可减少严重并发症的发生,有利于骨髓抑制的恢复。出现骨髓抑制后经综合治疗可有效恢复。目前神经母细胞瘤化疗方案发生严重骨髓抑制率较高,低剂量化疗作为新的化疗模式在神经母细胞瘤患者中有广泛的应用前景。

목적:탐토소인신경모세포류화료후엄중골수억제적림상표현급치료。방법분석산서성인동의원종2012년1월지2013년1월수치적12례소인신경모세포류화료소인기적엄중골수억제정황。결과12례환인균발생Ⅳ기골수억제,기중3례재교체화료과정중발생2차골수억제,균무엄중병발증,경치료2주내골수조혈공능회복정상。결론소인신경모세포류화료인기적골수억제,경조기발현가감소엄중병발증적발생,유리우골수억제적회복。출현골수억제후경종합치료가유효회복。목전신경모세포류화료방안발생엄중골수억제솔교고,저제양화료작위신적화료모식재신경모세포류환자중유엄범적응용전경。
Objective To analyze the clinical manifestations and treatment of myelosuppression after chemotherapy in pediatric neuroblastoma.Methods From January 2012 to January 2013 12 children with neuroblastoma were admitted in Shanxi Children ’s Hospital. The situation of myelosuppression caused by chemotherapy was analyzed .Results Stage Ⅳ myelosuppression occurred in all of these 12 cases, and myelosuppression occurred twice in 3 cases.There were no serious complications .Bone marrow hematopoiesis function came back to normal in 2 weeks.Conclusion If myelosuppression caused by chemotherapy of pediatric neuroblastoma can be detected early , the incidence of serious complications can be reduced , and it is beneficial to hematopoiesis function recovery .Myelosuppression can effectively recover after comprehensive treatment .Current ratio of myelosuppression caused by neuroblastoma chemotherapy is still high , so low dose chemotherapy , as a new mode of chemotherapy in patients with neuroblastoma , has broad application prospects .
