目的探讨同侧肾脏发病并且相对独立的透明细胞型和肾乳头状细胞癌的临床病理特点及免疫表型，提高对该肿瘤的认识和诊断水平。方法本研究回顾了2例病理诊断为透明细胞型合并肾乳头状细胞癌的临床资料，通过光镜和免疫组织化学染色，针对肾细胞癌相关蛋白标志物[包括 Vimentin、CD10、CK(AE1/AE3)、CK7、CK8/18、PAX2、PAX8、CAⅨ、AMACR]进行了观察和分析。结果2例患者为男性，年龄分别为70、63岁。2例患者的两处独立肿瘤均位于左侧肾脏，镜下观察均可见两处独立肿瘤，肿瘤间隔有正常肾脏组织，分别为乳头状肾细胞癌及透明细胞型肾细胞癌，且免疫组化显示2例患者肿瘤的表型一致。结论单侧肾脏肾透明细胞癌合并肾乳头状细胞癌是一种少见的临床现象，这种现象的存在以及类似的免疫组化表型提示透明细胞型肾细胞癌和乳头状肾细胞癌在发生过程中可能存在着内部的联系。
목적탐토동측신장발병병차상대독립적투명세포형화신유두상세포암적림상병리특점급면역표형，제고대해종류적인식화진단수평。방법본연구회고료2례병리진단위투명세포형합병신유두상세포암적림상자료，통과광경화면역조직화학염색，침대신세포암상관단백표지물[포괄 Vimentin、CD10、CK(AE1/AE3)、CK7、CK8/18、PAX2、PAX8、CAⅨ、AMACR]진행료관찰화분석。결과2례환자위남성，년령분별위70、63세。2례환자적량처독립종류균위우좌측신장，경하관찰균가견량처독립종류，종류간격유정상신장조직，분별위유두상신세포암급투명세포형신세포암，차면역조화현시2례환자종류적표형일치。결론단측신장신투명세포암합병신유두상세포암시일충소견적림상현상，저충현상적존재이급유사적면역조화표형제시투명세포형신세포암화유두상신세포암재발생과정중가능존재착내부적련계。
Objective To study the clinical pathological features of clear cell renal cell carcino-ma coexisted with papillary renal cell carcinoma in unilateral kidney. Methods 2 cases of renal clear cell carcinoma coexisted with papillary renal cell carcinoma were reviewed.The pathologic char-acters and immunohistochemistry results were analyzed.The primary antibodies for immunohisto-chemistry were Vimentin,CD10,CK (AE1/AE3 ),CK7,CK8/18,PAX2,PAX8,CA Ⅸ and AMACR. Results 2 cases were male.Age 70 years old,63 years old respectively.The two inde-pendent tumors of 2 cases were both located in the left kidney.Microscopically,the two independent tumors were clear cell renal cell carcinoma and papillary renal cell carcinoma respectively.2 cases of patients had similar immune-phenotype by immunohistochemistry results. Conclusions Clear cell renal cell carcinoma coexisted with papillary renal cell carcinoma is rare.It implies there might be some relationship between two types of kidney malignancies during their development.
