目的:探讨高效液相色谱法测定人尿微量白蛋白的临床应用价值。方法采集我科室自2012年10月至2013年10月进行尿微量白蛋白检测180例,分别进行高效液相色谱法和免疫比浊法进行临床对比分析,观察其检验结果。结果对尿液微量白蛋白行高效液相色谱法检测,出峰时间为1.7 min,而且非常稳定。检测低限是2 mg/L,有较满意的精密度和准确度。180例患者中,高效液相色谱法能检出96例微量白蛋白尿(检出率53.3%),免疫散射比浊法检出48例微量白蛋白尿(检出率26.7%),高效液相色谱法比免疫散射比浊法检出率更高。高效液相色谱法各份标本的结果均高于免疫散射比浊法。结论高效液相色谱法检测快速,流动相配制简单,灵敏度高,线性范围宽,结果准确,能检出更多的尿微量白蛋白,可以为糖尿病肾病的早期检测提供客观的科学依据,有较大的临床应用价值。

목적:탐토고효액상색보법측정인뇨미량백단백적림상응용개치。방법채집아과실자2012년10월지2013년10월진행뇨미량백단백검측180례,분별진행고효액상색보법화면역비탁법진행림상대비분석,관찰기검험결과。결과대뇨액미량백단백행고효액상색보법검측,출봉시간위1.7 min,이차비상은정。검측저한시2 mg/L,유교만의적정밀도화준학도。180례환자중,고효액상색보법능검출96례미량백단백뇨(검출솔53.3%),면역산사비탁법검출48례미량백단백뇨(검출솔26.7%),고효액상색보법비면역산사비탁법검출솔경고。고효액상색보법각빈표본적결과균고우면역산사비탁법。결론고효액상색보법검측쾌속,류동상배제간단,령민도고,선성범위관,결과준학,능검출경다적뇨미량백단백,가이위당뇨병신병적조기검측제공객관적과학의거,유교대적림상응용개치。
Objective To detect minute quality albumin in human urine by high performance liquid chromatography (HPLC). Methods One hundrend and eighty cases of minute quality albumin detection in human urine had been collected from October 2012 to October 2013. HPLC and immunoturbidimetry were performed respectively on these cases to contrast the testing results. Results The peak time of minute quality albumin in human urine by HPLC was 1.7 minute with stable feature. The detection lower limit was 2 mg/L with favorable precision and correctness. Ninty-six minute quality albumin cases were detected by HPLC in the 180 cases (detection rate=53.3%), whereas only 48 were detected by immunoturbidimetry (detection rate=26.7%). The detection rate results by HPLC was clearly higher than those by immunoturbidimetry. Conclusion HPLC has the advantages of rapid detection, simple mobile phase preparation, high sensitivity, wide linear range and reliable results. It can be applied to detect diabetes and kidney disorders at early stage , which provides objective basis for their clinical diagnosis.
