目的:探讨双套管结合负压吸引经皮肾镜I期治疗结石性脓肾的有效性。方法36例结石性脓肾患者均采用超声定位引导穿刺,一步法建立20 F经皮肾镜通道,经皮肾镜联合使用双套管结合负压吸引技术I期处理结石性脓肾。结果均成功建立20F经皮肾镜通道,行Ⅰ期碎石,手术时间为30~90 min,平均为45 min;未出现严重出血及感染性休克等并发症;术后4~5天复查KUB平片及彩超,29例患者结石一次性取净80.55%(29/36),结石残留7例(19.45%,7/36)。结论采用双套管结合负压吸引技术经皮肾镜取石术I期治疗结石性脓肾能保证碎石过程低压或者无压力,有效避免碎石过程中感染播散,是一种治疗结石性脓肾安全有效的方法。

목적:탐토쌍투관결합부압흡인경피신경I기치료결석성농신적유효성。방법36례결석성농신환자균채용초성정위인도천자,일보법건립20 F경피신경통도,경피신경연합사용쌍투관결합부압흡인기술I기처리결석성농신。결과균성공건립20F경피신경통도,행Ⅰ기쇄석,수술시간위30~90 min,평균위45 min;미출현엄중출혈급감염성휴극등병발증;술후4~5천복사KUB평편급채초,29례환자결석일차성취정80.55%(29/36),결석잔류7례(19.45%,7/36)。결론채용쌍투관결합부압흡인기술경피신경취석술I기치료결석성농신능보증쇄석과정저압혹자무압력,유효피면쇄석과정중감염파산,시일충치료결석성농신안전유효적방법。
Objective To investigate the effectiveness of the double casing pipe combined with suction technology in Phase I treatment of calculus pyonephrosis guided by nephroscope .Methods Using one -step method to puncture and establish 20F percutaneous nephrolithotomy channel for 36 patients those who suffered fromcalculus renal abscess guided by ultrasound , and Phase I treated calculus pyonephrosis by using double casing pipe combined with suction technology .Results 20F percutaneous nephrolithotomy channel for all patients were established successfully , phase one treat were done , the operative time was 30~90min, 45 min on average;none severe bleeding and septic shock and other complications occurred;4-5 days after the treatment KUB plain sheets and color doppler ultrasound were reviewed , 29 patients(80.55%,29/36) were stones free after a the treatment , and 7 cases with residual stones (19.45%,7/36).Conclusions The application of double casing pipe combined with suction technology was effectivein Phase I treatment of calculus pyonephrosis , for it could keep low/none pressure during the treatment and could avoid the spread of infection .
