目的改进我院不合理用药情况，促进合理用药的全面实施。方法采用回顾性调查方法，随机抽取我院2012年10月和11月门诊处方600张及100份出院病历的药品使用情况，分析其不合理用药现象，并探讨干预策略，进行逐一审查分析。结果绝大多数处方用药合理，不合理用药处方总计70张（占11.67％），主要表现为5个方面：联合用药品种过多、给药间隔时间及剂量不当、药理作用拮抗、抗菌药物使用不当、存在off-label用药行为。结论我院存在一定程度的不合理用药现象，临床医师应提高药学知识水平，医院药学应开发合理用药软件系统，以便更好地防治疾病、提高药物疗效、减少药物不良事件的发生。

Objective To improve the actuality of irrational use of drugs in our hospital and to promote the overall implement of rational use of drugs.Methods By using the method of retrospective review,random sampling the situation of 600 outpatient prescription and 100 the medical record in October and November 2012 in our hospital.The reasons of the irrational use of drugs were analyzed and the countermeasures were put forward by literature evaluation methods.Results The vast majority of the prescription is reasonable.The irrational prescriptions totaled 70(11.67%),mainly in five aspects as follow,the variety of drug combination is too much;the dosage and dosing interval is unsuitable;the pharmacological action antagonism;the improper use of antibacterials;off-label medication behavior existed.Conclusion The existence of irrational drug use in our hospital had be extent.Clinicians should raise the level of knowledge of pharmacy.Hospital pharmacy should be developed software system for rational use of drugs,to prevention and treatment of disease better,improving the efficacy of drugs and reduce the occurrence of adverse drug events.
