目的采用改良后方外括约肌皮下部切断联合内括约肌下缘松解术的方法对陈旧性肛裂的患者进行手术治疗并进行疗效分析。方法将100例陈旧性肛裂患者随机分为观察组和对照组，平均每组50例。观察组采用改良后方外括约肌皮下部切断联合内括约肌下缘松解术治疗；对照组采用常规侧位内括约肌切断术治疗。对两组患者的临床疗效、术后疼痛、创口愈合时间等指标进行观察分析。结果观察组治愈率94.00％。对照组治愈率82.00％，两组比较差异有统计学意义（P＜0.05）。观察组治疗后重度疼痛发生率5/50﹙10.00％﹚；对照组治疗后重度疼痛发生率10/50﹙20.00％﹚，两组有显著性差异﹙P＜0.05﹚。观察组治疗后创口愈合时间10～12d，平均11d；对照组治疗后创口愈合时间14～18d，平均16d。结论改良后方外括约肌皮下部切断联合内括约肌下缘松解术治疗陈旧性肛裂临床疗效显著，治愈率高、疼痛程度低、愈合时间短。值得在临床上推广和应用。
목적채용개량후방외괄약기피하부절단연합내괄약기하연송해술적방법대진구성항렬적환자진행수술치료병진행료효분석。방법장100례진구성항렬환자수궤분위관찰조화대조조，평균매조50례。관찰조채용개량후방외괄약기피하부절단연합내괄약기하연송해술치료；대조조채용상규측위내괄약기절단술치료。대량조환자적림상료효、술후동통、창구유합시간등지표진행관찰분석。결과관찰조치유솔94.00％。대조조치유솔82.00％，량조비교차이유통계학의의（P＜0.05）。관찰조치료후중도동통발생솔5/50﹙10.00％﹚；대조조치료후중도동통발생솔10/50﹙20.00％﹚，량조유현저성차이﹙P＜0.05﹚。관찰조치료후창구유합시간10～12d，평균11d；대조조치료후창구유합시간14～18d，평균16d。결론개량후방외괄약기피하부절단연합내괄약기하연송해술치료진구성항렬림상료효현저，치유솔고、동통정도저、유합시간단。치득재림상상추엄화응용。
Objective Patients with modified lower block sphincter skin cut joint internal sphincter lower edge of loose solution of clinical diagnosed with anal fissure were analyzed the effect of operation treatment. Methods Selected 100 patients with anal fissure patients were randomly divided into observation group and control group, 50 cases each of. The observation group using a modified automatic cut of the skin of lower sphincter with intersphincteric lower margin neurolysis for treatment of old anal fissure; the control group using conventional lateral internal sphincterotomy for chronic anal fissure. Two groups of patients for clinical cure rate, index of pain, wound healing time, postoperative comparative analysis. Results To observe the cure group and control group were 94% and 82%, there was significant difference between two groups (P < 0.05). Severe pain incidence of 5/50 after treatment (10%); severe pain incidence 10/50 control group after treatment (20%), there was significant difference between two groups (P < 0.05). The observation group after treatment, the wound healing time was 10 ~ 12D, average 11d; the control group after treatment, the wound healing time was 14 ~18D, average 16D. Conclusion Using the modified lower block sphincter cut of the skin with intersphincteric lower edge of loose solution on patients with clinical diagnosed with anal fissure by operation treatment effect is very obvious, has the advantages of the high cure rate, low degree of pain, healing time short, worthy of promotion in clinical application.
