目的比较MRI和US在子宫腺肌病中诊断价值,探讨两种方法对子宫腺肌病(AD)应用价值。方法对我院妇科门诊收集的82例疑似AD患者均行MRI和US检查。观察US和MRI表现,术后行病理学检查作为对比。结果82例疑似AD患者病理组织学检查,确诊子宫腺肌病38例,子宫肌瘤42例,子宫血管瘤2例。MRI与病理对照后,确诊子宫腺肌病35例,误诊1例,漏诊2例,诊断符合率92.1%,误诊率2.6%,漏诊率5.3%;US与病理对照后,确诊子宫腺肌病31例,误诊4例,漏诊7例,诊断符合率81.6%,误诊率10.5%,漏诊率18.4%,二者对比差异有统计学意义(P<0.05)。结论 MRI诊断子宫腺肌病敏感性、特异性、准确性明显优于US,且对子宫结构显示更为清晰,还可进行定性、定位诊断,临床实用价值更突出。

Objective This study is to compare the diagnostic value of MRI and US in adenomyosis and discuss the application value of these two methods in adenomyosis (AD).Method 82 suspected AD patients in the gynecological clinic of our hospital were collected to have MRI and US. US and MRI manifestations of these patients were observed and the postoperative pathological examinations were compared. Results 82 suspected AD patients underwent histopathologic examination, of which 38 patients were diagnosed with adenomyosis, 42 patients with hysteromyoma and 2 patients with hemangioma of uterus. Through the comparison with MRI, 35 patients were diagnosed with adenomyosis, 1 case was misdiagnosed and 2 cases were missed, with the diagnostic accordance rate of 92.1%, misdiagnosis rate of 2.6% and missed diagnosis rate of 5.3%. Through the comparison with US, 31 patients were diagnosed with adenomyosis, 4 cases were misdiagnosed and 7 cases were missed, with the diagnostic accordance rate of 81.6%, misdiagnosis rate of 10.5% and missed diagnosis rate of 18.4%. The difference was statistically significant (P<0.05).Conclusion MRI is superior to US in the diagnosis of adenomyosis in sensitivity, specialty and accuracy, with clearer uterus structure display. Moreover, MRI can be used in qualitative diagnosis and localization diagnosis, indicating more prominent clinical application value.
