《国家基本公共卫生服务规范（2011年版）》实施以来，基层医疗卫生机构原有门诊日志的项目设计已不能满足全科医学工作的需要。基层医疗卫生机构传染病疫情发现和报告的原始记录存在于门诊日志和居民健康档案中，其门诊日志项目应为：服务日期、姓名、性别、年龄、职业、工作单位（学生填写学校）、联系电话、现住址（健康档案号）、病名（健康问题）、发病日期、初诊或复诊、血压（左右上臂）或血糖、处理13项内容。
