目的:对比研究超声弹性成像(UE)与乳腺影像学报告及数据系统(BI-RADS分级)在乳腺癌诊断方面的应用价值。方法:将病理证实的78例乳腺肿瘤患者(共119个病灶)作为研究对象。对所有患者分别行UE及BI-RADS分级检查。在UE中以≥4分为诊断恶性的标准;在BI-RADS分级中以≥4级为诊断恶性的标准。结果:UE诊断乳腺癌的敏感性89.5%、特异性88.4%、准确性89.1%,约登指数为0.779;BI-RADS分级诊断乳腺癌的敏感性76.3%、特异性44.2%、准确性64.7%,约登指数为0.205。UE的约登指数显著高于BI-RADS分级(P=0.000)。结论:在乳腺癌的诊断中UE的诊断价值高于BI-RADS分级,在临床应用中若将两者结合起来检查,可能会大大提高乳腺癌诊断的准确性。

목적:대비연구초성탄성성상(UE)여유선영상학보고급수거계통(BI-RADS분급)재유선암진단방면적응용개치。방법:장병리증실적78례유선종류환자(공119개병조)작위연구대상。대소유환자분별행UE급BI-RADS분급검사。재UE중이≥4분위진단악성적표준;재BI-RADS분급중이≥4급위진단악성적표준。결과:UE진단유선암적민감성89.5%、특이성88.4%、준학성89.1%,약등지수위0.779;BI-RADS분급진단유선암적민감성76.3%、특이성44.2%、준학성64.7%,약등지수위0.205。UE적약등지수현저고우BI-RADS분급(P=0.000)。결론:재유선암적진단중UE적진단개치고우BI-RADS분급,재림상응용중약장량자결합기래검사,가능회대대제고유선암진단적준학성。
Objective:To compare the value of ultrasonic elastography (UE) and breast imaging report and data system(BI-RADS) in the differential diagnosis of breast carcinoma.Method:Seventy-eight patients with breast solid lesion were enrolled in this study. All the patients were examined by breast imaging report and data system(BI-RADS)and ultrasonic elastography(UE). In the UE examination,the results greater than or equal to 4 are diagnosed with malignance;in the BI-RADS classification,level greater than or equal to 4 was standard for the diagnosis of malignance.Result:The sensitivity,specificity,accuracy and Youden index of UE were 89.5%,88.4%,89.1%and 0.779 respectively,and those of BI-RADS were 76.3%,44.2%,64.7%and 0.205,respectively. The Youden index of UE was significantly higher than that of BI-RADS(P=0.000).Conclusion:In the diagnosis of breast cancer,the diagnostic value of UE is higher than BI-RADS. If we combine the two methods to check patients in clinical applications,we may greatly improve the accuracy of diagnosis of breast cancer.
