目的探讨急性百草枯中毒患者的临床特点,以寻找降低病死率的对策.方法对我院2008年1月至2012.1月收治的14例重度急性百草枯中毒患者的临床表现和治疗方法进行回顾性分析.结果14例患者中死亡9例,病死率高达64.3%,7例死于医院,中毒至死亡时间为40h-175h,2例放弃治疗出院后死亡,死亡原因均为多脏器功能衰竭.存活患者5例,均及时给予催吐、洗胃和导泻等治疗和支持治疗,4例表现为非少尿型急性肾衰竭.结论急性百草枯中毒后应尽早催吐、洗胃和导泻,减少毒物吸收,血液净化和免疫制剂的应用在降低重症患者死亡率上未见明显优势.

Objective To analysis clinical characteristics of the patients with acute paraquat poisoning, find strategies to reduce the mortality rate . Method A total of 14 patients subjected to acute paraquat poisoning admitted from January 2008 to January 2012 were retrospectively studied. Results The mortality was 64.3%(9 cases),and survival time ranged from 40h to 175h.7 cases died inhospitals,2 cases die athome, and all of them died from multiorgan failure.5 cases who were given gave emetic, gastric lavage, catharsis and symptomatic treatment survived.4 cases of survivors were complicated with non oliguria acute renal failure. Conclusions The patients with acute paraquat poisoning should be given emetic, gastric lavage and catharsis as soon as possible to reduce absorption of the toxic chemical after poisoning. Blood purification and immunosuppressive therapyhad no remarkable superiority in decreasing mortality of critical patients.
