目的探讨胎儿脐静脉脱细胞基质作为尿道缺损修补替代材料的可行性.方法选择24只雄性白兔,随机分为实验组(n=18)和对照组(n=6).实验组切除尿道2.0cm后用人脐静脉细胞外基质修复,对照组离断长2.0cm的尿道后直接缝合两断端,术后行组织学和膀胱尿道造影评价尿道修复情况及功能.结果实验组中有1只出现尿瘘,对照组4只出现尿瘘,2只排尿困难.术后16周实验组尿道膀胱造影实验组见尿道完整、光滑,无尿液外渗、尿道憩室等形成.对照组尿道缺损处管腔变窄,无法注入造影剂.术后2周实验组移植段尿道有上皮细胞生长,8周时移植段出现不规则紊乱的平滑肌细胞生长,无炎性细胞;16周后,新尿道上皮下方为正常的尿道海绵体组织,无明显的炎性细胞浸润.结论胎儿脐静脉脱细胞基质是一种较为理想的管状尿道修补替代材料.

Objective To investigate the feasibility of urethral reconstruction w ith acellular tissue matrices of umbilical vein. Methods Twenty-four male rabbits were divided randomly into two groups,,including experimental and control group. In experimental group of 18 rabbits ,the resected urethral of 2.0cmwas repaired with acellular tissue matrices of umbilical vein. The urethra of regeneration segment was studied by histological technique and retrograde urethrography after operation. Results Two weeks after operation ,epithelial cells were migrated on the new formed urethras. At 8th week after operation, new smooth-muscle cells were confirmed. At 16th week postoperatively, Histological examination showed complete epithelialization of the urethras w ith multi- layers of epithelium ce lls and no apparent inflammatory cells infiltrated in the spongy tissues below. Conclusion Acellular umbilical vein graft might be an ideal tissue eng ineering material for urethral reconstruction.
