目的观察血必净注射液对慢性阻塞性肺疾病(COPD)合并呼吸衰竭的临床疗效及对患者血清肿瘤坏死因子(TNF-α),C反应蛋白(CRP)等炎性因子水平的影响.方法将58例COPD合并Ⅱ型呼吸衰竭的患者随机分为对照组和治疗组,每组29例.两组患者均接受有创机械通气及常规药物治疗,治疗组加用血必净注射液.监测治疗前后血气分析、血清TNF-α、CRP的变化,并观察在10 d内撤机成功率及脱机时间.结果治疗后治疗组PaO2、PaCO2均有明显改善,与治疗前比较,具有显著性差异(P<0.05).两组治疗后PaO2比较,差异也具有显著性意义(P<0.01).治疗组血清TNF-α,CRP均较治疗前明显改善,差异有统计学意义(P<0.05).治疗组治疗前后TNF-α,CRP的变化程度较对照组更明显.差异有统计学意义(P<0.05).治疗组平均脱机时间比对照组明显缩短,差异有统计学意义(P<0.05).结论血必净注射液治疗COPD合并呼吸衰竭疗效显著,有助于减轻患者TNF-α、CRP介导的炎性反应,减少呼吸机依赖,提高COPD危重患者的救治成功率.

To explore the effect of Xuebijing injection on serum inflammatory factory such as tumor necrosis factory-alpha(TNF-α) and C-reactive protein(CRP) in patients with respiratory failure complicated by chronic obstructive pulmonary disease. Methods Totally 58 COPD patients were randomly divided into control group and treatment group with 29 patients in each group. The control group received conventional treatment and mechanical ventilation.The treatment group combined with Xuebijing treatment. The changes of blood gas analysis,serum TNF-αand CRP were monitored. The invasive ventilation time were also recorded in two groups. Results Compared with before treatment ,serum levels of TNF-αand CRP in treatment group were obviously improved, and there was significant difference(P<0.05). Compared with the control group, the changes of TNF-αand CRP in treatment group before and after treatment were more obvious, and there was significant difference(P<0.05). The invasive ventilation time of treatment group was significantly shorter compared with control group. Conclusions Xuebijing injection for the treatment of respiratory failure complicated by chronic obstructive pulmonary disease has significant efficacy, and is helpful for reducing the inflammatory responses induced by TNF-αand CRP. Also is helpful for reducing the ventilator dependence.
