目的探讨以体温作为阿托品化的定量指标抢救急性有机磷农药中毒(AOPP)的具体方法和价值.方法88例AOPP患者被随机分为治疗组和对照组.将体温分为四个阶段:<37℃;37℃~37.5℃;37.5℃~38℃;>38℃.给药策略:T<37℃,提示阿托品不足,给予大剂量阿托品;T升至37℃~37.5℃提示开始阿托品化,减量给予阿托品;T37.5℃~38℃,提示已阿托品化,大幅度减少阿托品用量;T>38℃提示已阿托品过量中毒,需暂停阿托品,待T降至37℃~37.5℃再开始给小剂量阿托品;若降至<37℃,重新给予较大剂量阿托品.结果该方法抢救AOPP的死亡率为6%,对照组为21%.结论以体温作为阿托品化的定量指标抢救AOPP,方法简便易行,能明显降低阿托品过量中毒和不足的死亡率.值得推广应用.

Objective To explore the specific methods and value. of temperature as atropinizaton quantitative index in treatment of acute organophosphorus pesticide poisoning (AOPP) Methods The 88 AOPP patients were randomly divided into treatment group and control group. The temperature was divided into:four stages: < 37 ℃; 37℃~37.5℃; 37.5℃~38℃; > 38℃..Dosing method: T < 37℃, indicating given atropine shortage ,to give larger doses of atropine; T rised to 37℃~37.5℃, indicating the atropinization had,been started,to reduce doses of atropine;T to 37.5℃~38℃, indicating atropinzation had been reacded, to greatly reduce the dosage of atropine;T > 38℃, indicating atropine overdose poisoning, to suspend the use of atropine, wait for T drops to 37℃~37.5℃ and began to give small doses of atropine;If T fell to <37℃, the larger doses of atropine are given again. results The method was used to treat the AOPP with a mortality rate of 6% in the therapy group, 21% in the control group. Conclusion the method on the temperature as atropine quantitative index in treatment of AOPP is simple and easy to operate, and can significantly reduce the mortality rate of overdose poisoning and insufficient dosage of atropine . It is popularization and application.
