目的分析及评价应用人工关节置换治疗高龄股骨粗隆间骨折的方法及疗效.方法应用人工关节治疗70岁以上股骨粗隆间骨折24例,男9例,女15例,年龄70~87岁,平均76.5岁.骨折类型按Evans分类：Ⅲ型3例,Ⅳ型14例,Ⅴ型7例.结果术后3~20d下床,平均14天；住院时间16~30天,平均20天.24例患者随访0.5~8年(以髋关节活动度、疼痛程度及生活自理情况等为观察指标),优良率为87.5%.无一例死亡,未出现假体松动、下沉等现象.结论人工假体置换手术是高龄股骨粗隆间骨折治疗方法之一,近期疗效满意,但远期效果仍需继续观察.

Objective TO analysis and assess the curative effect and method of artificial total hip replacement on intertrochanteric fracture in senior patients. Methods 24 cases of intertrochanteric fracture above 70 years old in our department adopt artificial total hip replacement, male 9 cases, female 15 cases , from 70-87 years olds , average 76.5 years olds. According to Evans classification, Ⅲ type 3 cases , Ⅳtype 14 cases, Ⅴtype 7 cases. Results The patients walked with stick and with load gradually 3--20 days(mean14days) after operation. The hospital length of stay is from 16 to 30 days , the average hospital length of stay(LOS)is 21 days. And 16 cases were followed up for half 2eight years. And we adopt hip mobility, pain and independent living conditions for observation index, excellent is87.5 %, no deaths , no faseptic loosening and subsidence. Conclusions total hip replacement is the an effective method for intertrochanteric fracture in senior patients, short-term therapeutic effect is perfect, but long-term results is still need to observe.
