目的分析总结外伤性脑疝前驱期特征性临床征象及意义.方法回顾性分析2005年1月~2012年3月我院脑外科科收治的137例患者脑疝发生前2 h内的临床特点和辅助检查资料，并与同期治疗的140例未发生脑疝的重型颅脑损伤患者做对比.结果脑疝组前驱期征象如昏迷加深、烦躁加重、尿失禁、侧脑室和外侧裂受压变窄伴脑基底池受压变窄、生命体征改变具有特征性，其发生率明显高于非脑疝组，两组差异有显著性意义(P<0.05).结论颅脑损伤患者出现脑疝前驱期特征性临床征象是超早期开颅手术的特殊的、迫切的手术指征.

Objective To analyze and summarize the traumatic brain hernia in prodromal period the characteristic clinical sign and significance. Methods a retrospective analysis of 2008 January to 2012 March in our hospital department of cerebral surgery was performed in 137 cases of patients with brain hernia occurred within 2 h and the clinical features of accessory examination, and with the same period in treatment of 140 patients with cerebral hernia has not occurred in patients with severe head injury compared. Results cerebral hernia group prodromal period indicia such as coma deepened, irritability, increased urinary incontinence, lateral ventricle and lateral fissure compression narrows with basilar pool compression narrows, changes of vital signs are characteristic, its incidence was significantly higher than that of brain hernia group, the difference between the two groups was significant ( P<0.05 ). Conclusion traumatic brain injury patients with herniation of the prodromal period the characteristic clinical sign is ultra early craniotomy operation special, urgent operation indication.
