重症急性胰腺炎(SAP)作为临床常见的急危重症之一，观念从单纯性西医保守治疗向中西医结合保守治疗转变。该文对近年来西医保守治疗和中西医保守治疗进展作了综述，在西医保守治疗方面包括存在争议的几个常规治疗，如镇痛、抑制胰腺外分泌和胰酶抑制剂应用、预防性抗生素治疗、营养支持等，及近年来出现的治疗新观念；中西医结合治疗方面包括在西医常规治疗基础上运用经方加减及单味中药的不同给药途径，使SAP病程明显缩短，体现了其治疗SAP的优势。

Astract]Severe acute Pancreatitis (SAP) is one of a common severe clinical disease.The medical treatment has shifted from a pure western medicine therapy to an integrated Chinese and Western medicine conservative therapy in curing SAP. This review summarized the progress of the SAP treatment with western medicine and integrated Chinese and Western medicine in recent years.There are some existing debates about conventional management including analgesia,inhibition of pancreatic exocrine secretion , application of trypsin inhibitor , prophylactic antibiotics usage , nutrition support and the new concept and approache on western medicine conversative therapy.The “integrated traditional Chinese and Western medicine conservation treatment”is built on the foundation of conventional western medicine therapy,combined with the decodtion which can plus or clincical and the multi-channel application of single tradional Chinese herb.This varies applications of using integrated Chinese and Western medicine therapy in curing SAP has proven to be effective.The advantages of adapting to integrated traditional Chinese and Western medicine have significantly shortened the duration of SAP and its prevailing adwantages of future appications.
