目的探讨心电图检测对诊断高钾血症的临床价值.方法回顾性分析我院2009年3月至2011年5月高血钾患者25例的心电图资料,并对心电图与血清钾浓度进行对比分析.结果血清钾5.5-6.9mmol/L时主要表现为T波振幅增高,基底部变窄,波顶变尖,呈帐篷状,与实验室诊断符合率为34.2%;血清钾>7.0mmol/L时,P波振幅降低或消失、P-R间期延长、QRS时限增宽、R波振幅减低、S波加深、S T段压低、Q T间期延长、室内束支或分支传导阻滞,甚至出现窦性心动过速和度房室传导阻滞等,与实验室诊断符合率为86.7%.结论心电图表现与血清钾测定有较好的一致性,它是一种简单、快速诊断高血钾的有效方法,为临床及时诊断及治疗提供重要的依据.

Objective To investigate the ECG diagnosis of hyperkalemia clinical value. Methods A retrospective analysis of ECG data of 25 cases of hyperkalemia patients in our hospital from March 2009 to May 2011, and electrocardiogram and serum potassium concentration were analyzed. Results mainly serum potassium 5.5-6.9mmol / L for the T-wave amplitude increased, the narrow base of the wave top is tapered, was the tent-like rate of 34.2%, in line with the laboratory diagnosis; Serum potassium> 7.0mmol / L, P wave amplitude decreased or disappeared, PR interval prolongation, QRS duration widened, R wave amplitude to reduce the S wave and deep ST segment depression, QT prolongation, the interior bundle branch or fascicular block, even sinus tachycardia and atrioventricular block with laboratory diagnosis was 86.7%. Conclusion ECG performance and serum potassium determination better consistency, it is a simple, fast diagnosis hyperkalemia effective method, provide an important basis for the clinical timely diagnosis and treatment.
