目的探讨脉压指数(PPI)与原发性高血压患者左心室舒张功能损害程度的关系.方法162例经外周肱动脉压力测定收缩压(SBP)、舒张压(DBP),以PPI≤0.40、>0.40分组,2组进行分析.比较2组左心形态、左心室收缩功能和舒张功能情况.结果 PPI>O.40组左心房内径明显增大(P<0.001);左心室内径无明显改变(P>0.05);室间隔、左心室后壁明显增厚,E/A值降低(P<0.001),而左心室射血分数差异无显著性(P>0.05).结论对于原发性高血压患者左心室舒张功能异常出现早于左心室收缩功能异常,PPI>0.40提示高血压患者早期合并有舒张功能异常.

Objective To study the relationship between pulse pressure index(PPI)and left ventricular diastolic function.Methods 162 patients, whose systolic blood pressure(SBP)and diastolic blood pressure(DBP) were measured through the peripheral humeral artery, were divided into two groups according to the PPI(PPI≤0.40 or PPI>0.40). The geometrical figure of the left ventricle atrium and the systolic and diastolic function between two groups were compared .Results In the group(PPI>0.40),the average diameter of the left atria was significantly larger(P<O.001),and the ventricular septum and the posterior wall were significantly thicker(P<0.001).But the ejection fraction of the left ventricular had no significant difference between two groups.Conclusion The patients with essential hypertension often had left ventricular diastolic dysfunction before the ventricular systolic function Was impaired, and PPI>0.40 suggested the left ventricular diastolic dysfunction in early stage of hypertension.
