目的:探讨数字化模板测量在发育性髋节发育不良的全髋关节置换术中的应用效果。研究数字化模板测量在假体选择上的精确性、可靠性及在均衡双下肢长度中作用。方法自2011年1月-2012年12月,对36例发育性髋节发育不良的患者进行全髋关节置换术。术前通过数字化模板测量预测假体的类型、型号、位置、旋转中心及股骨颈截骨高度、双下肢长度,与术后实际应用相比较,进行统计分析。结果在36例发育性髋节发育不良患者进行全髋关节置换术中,髋臼假体符合优良率率75%,股骨假体符合优良率83.3%,71.8%的患者恢复双下肢长度。术前与术后相比较,旋转中心在水平距离上无统计学差异(P>0.05),垂直距离上有统计学差异(P<0.05)。结论应用数字化模板进行术前设计,在发育性髋节发育不良的全髋关节置换术中具有较高的准确率和可信度,对于均衡双下肢长度具有一定的参考价值。

Objective To discuss the utility of digital templating in total hip arthroplasty with developmental dysplasia of the hip ( DDH) .To research the accuracy and reliability of digital templating in prosthesis size selection and balanced the length of the lower limbs.Methods Since January 2011 to December 2012, 36 patients have total hip arthroplasty with developmental dysplasia of the hip.Through digital templating forecast prosthesis size , location and femoral neck osteotomy height before the THA .After surgery,com-pared with the practical application of the prosthesis size for statistical analysis .Measuring the actual length of lower limbs after surger-y.Results In 36 cases of DDH patients with total hip replacement ,acetabulum prosthesis accord in the excellent rate was 75%,the femoral prosthesis accord in the excellent rate was 83.3%,71.8% of the patients was to restore the length of lower limbs .Preoperative and postoperative,center of rotation in the horizontal distance have no statistical difference (P>0.05),the vertical distance was statis-tically difference (P<0.05).Conclusion Using digital templating measurement for preoperative design , in total hip arthroplasty with developmental dysplasia of the hip has the high accuracy and credibility .For the balance of lower limbs length has a certain reference value.
