目的:通过观察80例病人鼻息肉组织中炎性细胞的分布情况,探寻以嗜酸性粒细胞数目作为评估鼻息肉患者预后及治愈时间的指标的可行性。方法共80份鼻息肉标本,每份标本切片五张,HE染色,采用效果较好一张计数,每张HE染色切片随机选5个高倍镜视野(×400),手工计数嗜酸性粒细胞、中性粒细胞、淋巴细胞的数目,取其平均值,并进行统计学分析。同时,根据患者鼻息肉组织中嗜酸性粒细胞数目,分为嗜酸性粒细胞浸润组和非嗜酸性粒细胞浸润组,统计分析两组中嗜酸性粒细胞数、淋巴细胞数、中性粒细胞数。回顾性分析两组中患者的治愈好转率、治愈时间。结果两组间嗜酸性粒细胞数、淋巴细胞数、中性粒细胞数差异均有统计学意义。两两比较后发现,在嗜酸性粒细胞浸润型组织中,嗜酸性粒细胞最多,淋巴细胞次之,中性粒细胞最少。在非嗜酸性粒细胞浸润型组织中淋巴细胞最多,嗜酸性粒细胞次之,中性粒细胞多于嗜酸性粒细胞浸润型。嗜酸性粒细胞浸润型患者治愈好转率明显低于非嗜酸性粒细胞浸润型,治愈时间明显长于非嗜酸性粒细胞浸润型。嗜酸性粒细胞数与治愈时间呈正相关。结论大部分鼻息肉患者的病理组织是以嗜酸性粒细胞浸润为主,表明嗜酸性粒细胞在鼻息肉发病机制中发挥重要的作用。且嗜酸性粒细胞越多,治愈时间越长。同时嗜酸性粒细胞可作为评估鼻息肉患者预后及治愈时间的参考指标。

목적:통과관찰80례병인비식육조직중염성세포적분포정황,탐심이기산성립세포수목작위평고비식육환자예후급치유시간적지표적가행성。방법공80빈비식육표본,매빈표본절편오장,HE염색,채용효과교호일장계수,매장HE염색절편수궤선5개고배경시야(×400),수공계수기산성립세포、중성립세포、림파세포적수목,취기평균치,병진행통계학분석。동시,근거환자비식육조직중기산성립세포수목,분위기산성립세포침윤조화비기산성립세포침윤조,통계분석량조중기산성립세포수、림파세포수、중성립세포수。회고성분석량조중환자적치유호전솔、치유시간。결과량조간기산성립세포수、림파세포수、중성립세포수차이균유통계학의의。량량비교후발현,재기산성립세포침윤형조직중,기산성립세포최다,림파세포차지,중성립세포최소。재비기산성립세포침윤형조직중림파세포최다,기산성립세포차지,중성립세포다우기산성립세포침윤형。기산성립세포침윤형환자치유호전솔명현저우비기산성립세포침윤형,치유시간명현장우비기산성립세포침윤형。기산성립세포수여치유시간정정상관。결론대부분비식육환자적병리조직시이기산성립세포침윤위주,표명기산성립세포재비식육발병궤제중발휘중요적작용。차기산성립세포월다,치유시간월장。동시기산성립세포가작위평고비식육환자예후급치유시간적삼고지표。
Objective To explore and the feasibility of assessing the prognosis and healing time of patients by observing the distri -bution of eosinophils of nasal polyps .Methods The nasal polyps of each 80 patient were stained with HE to count the number of eosin-ophils, lymphocytes and neutrophils under a light microscope and were divided into 2 groups according to the number of eosinophils:①eosinophil infiltration and ②noneosinophil infiltration ,counting the number of three kinds cells in two groups .Retrospective analysis the cure rate and time to heal in the two groups improved .Results There was significant difference of the mean number of eosinophils , lymphocytes and neutrophils (P<0.05).The difference of the mean number of eosinophils among these years was significant (P<0.05) and presented a uptrend year by year .The difference of the mean number among eosinophils ,lymphocytes and neutrophils in two groups was significant .In the eosinophils infiltrating , eosinophils were found predominantly in the group of eosinophil infiltration , lym-phocytes taking the second place , neutrophils the least .In the other group of noneosinophil infiltration , lymphocytes were found pre-dominantly , eosinophils the second , neutrophils in this group was more than the other group .The recovery rate of nasal patients with nasal polyps in group of eosinophil infiltration was significantly inferior to the group of noneosinophil infiltration , also the curative time was sharply longer .The number of eosinophils was positive correlated with the curative time .Conclusion The number of eosinophil infiltration was much more than noneosinophil ˊs.Eosinophils play an important role in the pathogenesis of nasal polyps .The more eo-sinophils infiltrate in the nasal polyps , the longer cure time it takes .And eosinophils index can be used to evaluate the prognosis and healing time of patients with nasal polyps after functional endoscopic sinus surgery .
