目的：研究传统龈下刮治及根面平整的基础上联合应用Nd ：YAG激光治疗侵袭性牙周炎的短期临床疗效及镇痛效果。方法通过选择34例侵袭性牙周炎患者，以龈上洁治后1周为基线，采用自身对照，随机选择左右侧作为A组（对照组）单纯龈下刮治及根面平整组和B组（实验组）龈下刮治及根面平整＋Nd ：YAG激光治疗组。术后即刻应用视觉模拟评分法（visual analogue scale ，VAS）让患者对治疗产生的疼痛度进行评分；基线和治疗后1月、3月由另一名单盲牙周医师进行牙周指数的测量，包括：牙周探诊深度（probing depth ，PD）、临床附着丧失（clinical attachment loss ，CAL）、出血指数（bleeding index ，BI）和牙齿松动度。结果：激光治疗组的VAS值（1．66±0．74）明显低于对照组（4．81±0．59），p＜0．01；激光治疗组和对照组患牙PD、CAL及BI指数及牙齿松动度于治疗后1月、3月相对于基线均有明显降低，激光组的PD和CAL指数在治疗后3月相对于治疗后1月有持续降低，明显低于对照组，p＜0．05。结论 Nd ：YAG激光与常规龈下刮治及根面平整术相结合能有效提高侵袭性牙周炎的短期临床疗效，同时改善治疗的舒适性。
목적：연구전통간하괄치급근면평정적기출상연합응용Nd ：YAG격광치료침습성아주염적단기림상료효급진통효과。방법통과선택34례침습성아주염환자，이간상길치후1주위기선，채용자신대조，수궤선택좌우측작위A조（대조조）단순간하괄치급근면평정조화B조（실험조）간하괄치급근면평정＋Nd ：YAG격광치료조。술후즉각응용시각모의평분법（visual analogue scale ，VAS）양환자대치료산생적동통도진행평분；기선화치료후1월、3월유령일명단맹아주의사진행아주지수적측량，포괄：아주탐진심도（probing depth ，PD）、림상부착상실（clinical attachment loss ，CAL）、출혈지수（bleeding index ，BI）화아치송동도。결과：격광치료조적VAS치（1．66±0．74）명현저우대조조（4．81±0．59），p＜0．01；격광치료조화대조조환아PD、CAL급BI지수급아치송동도우치료후1월、3월상대우기선균유명현강저，격광조적PD화CAL지수재치료후3월상대우치료후1월유지속강저，명현저우대조조，p＜0．05。결론 Nd ：YAG격광여상규간하괄치급근면평정술상결합능유효제고침습성아주염적단기림상료효，동시개선치료적서괄성。
Objective :To study the short -term clinical effects and analgesic effect of periodontal treatment combiding Nd :YAG laser with subgingival scaling and root planing (SRP)for patients with aggressive periodontitis .Methods 34 patients with aggressive periodontitis were selected .Teeth on left or right side are randomly treated with SRP(control group) ,or SRP combiding with Nd :YAG laser (test group ) .The degree of discomfort experienced during the treatment was graded by the patient using visual analogue scale (VAS) immediately after the completion of treatment .The following periodontal indexes were recorded by a mono-blinded examiner :probing depth(PD) ,clinical attachment loss (CAL) ,bleeding index (BI) ,and tooth mobility .Results The mean of VAS values of the test group(1 .66 ± 0 .74)was significantly lower than control group (4 .81 ± 0 .59 ) ,p<0 .01 .Both the groups showed significant reduction of PD ,CAL ,BI and tooth mobility values 1month and 3 months after treatment .3 months later ,the PD ,CAL values of the test group was continuely decreasing ,and was significant lower than test group ,p<0 .05 .Conclusion The combination of Nd :YAG laser with SRP could improve the short -term clinical effects and comforbility for patients with aggressive periodontitis .
